Which food services passed or failed their health inspection: Aug. 25 – Aug. 31

SYRACUSE, N.Y. (WSYR-TV) — The Onondaga County Health Department released its inspection reports for restaurants checked from August 25 through August 31.
Three food services failed, and one received an unsatisfactory inspection
Failures:
Cloud Nine Creamery and Cafe – 8202 Brewerton Road, Cicero
Denny’s – 103 Elwood Davis Road, North Syracuse
Ichiban Hibachi Steakhouse – 103 Elwood Davis Road, North Syracuse
Unsatisfactory:
Kabul Restaurant – 701 South Crouse Avenue, Syracuse
Cloud Nine Creamery and Cafe
This restaurant received four violations, with one in critical condition.
Foods adulterated or received from unapproved sources (critical): The inspector found coneys, turkey deli meat, and two tomatoes, spoiled in the preparation top portion of the deli cooler in the cafe side. This was corrected and approximately six coneys, a pound of deli meat and two tomatoes were thrown out.
Improper sanitary facilities and controls: The inspector found the floor drains below the three-bay sink with some buildup and not maintained.
Improper sanitary facilities and controls: The inspector found the handwash sink in the kitchen lacked single-service paper towels. This was corrected during the inspection.
Inadequate insect/rodent control: The inspector found mouse droppings on the floor around the hot water heater and furnace, on the top of canned goods and containers in the dry storage area. Droppings were also noted on the floor around the deli cooler in the front cafe area. Small black flies were found in the bathroom, around drains and in cafes.
Denny’s
This restaurant received 12 violations, with three in critical condition.
Improper cooling and refrigerated storage of potentially hazardous foods: The inspector found approximately 100 raw whole-shell eggs on the cook line in the egg cooler at 56 to 72 degrees Fahrenheit. It was undetermined how long they were out of the temperature. Additionally, one quart of liquid pasteurized egg product was found in the exterior portion of the same egg cooler at 60 degrees Fahrenheit. It was also undetermined as to how long they were out of temperature.
This was corrected and the whole and liquid eggs were thrown out.
Improper cooling and refrigerated storage of potentially hazardous foods: The inspector found at the service aisle wait station, approximately 100 half and half dairy creamers at 65 to 72 degrees Fahrenheit. According to management, the products were stored in a large container for an unknown amount of time at room temperature. This product requires refrigeration and was purchased because the shelf-stable product they usually have was unavailable. The staff was unaware that refrigeration was necessary. The creamers were all voluntarily thrown out.
Improper hot holding of potentially hazardous foods : The inspector found on the cook line, approximately one quart of white country gravy at 122 degrees Fahrenheit in the hot holding unit. An employee said it was removed from the walk-in cooler about one hour before, and was reheated in the microwave to an undetermined temperature. The water level of the hot holding unit was found to be lower than the bottom of the pans. This was corrected and the product was removed and reheated in a metal pan at 165 degrees Fahrenheit. It was brought back to the hot holding unit with additional water added to the hot holding unit.
Food not protected in general: The inspector found ice cream scoops and spoons in standing water in a dipper well at the wait station at 127 degrees Fahrenheit instead of 140 degrees Fahrenheit minimum as required.

Food not protected in general: The inspector found in the dry storage area, unused coffee filters with brown stains. They were voluntarily thrown out.
Improper cleaning, washing and sanitizing of equipment and utensils: The inspector found cook line equipment control panels, all cook line shelving, dry storage shelving, walk-in cooler shelving, and interior of microwave ovens all had food soil. The bottom of the ice cream chest freezer had food debris.
This violation was found four times.

Ichiban Hibachi Steakhouse
This restaurant received 19 violations, with three in critical condition.
Foods not protected from contamination, temperatures not measured : The inspector found the establishment does not have any metal stem type analog or digital type food thermometers to use in evaluating food temperatures. This was corrected and the restaurant was loaned a working digital food thermometer and a digital-analog metal stem thermometer is to be purchased. The loaned thermometer will be returned at reinspection.
Improper cooling and refrigerated storage of potentially hazardous foods: The inspector found approximately 10 lbs. of cooked rice in a large tub on the preparation table. The rice had a temperature of 55 to 58 degrees Fahrenheit. According to management, the rice had been sitting out of refrigeration for about 30 to 40 minutes after some was used. This was corrected and the product was returned to the walk-in cooler.
43 whole raw shell eggs were found sitting in the flat carriers at 55 to 57 degrees Fahrenheit. According to management, the eggs had been out of refrigeration for about 20 to 25 minutes after some were used. This was corrected and the eggs were returned to the walk-in cooler.
This violation was found two times.
Food not protected in general: The inspector found many squeeze bottles not labeled with their contents. The ceramic cup was being used as a scoop in raw ice and lacked a handle.
Poor sanitary design, construction, installation of equipment and utensils : The inspector found several large and small poly cutting boards in poor repair with heavy staining and numerous cut marks and worn surfaces.
This violation was found two times.

Improper cleaning, washing and sanitizing of equipment and utensils: The inspector found a food scale not in use was not clean and had food soil.
Improper cleaning, washing and sanitizing of equipment and utensils: The inspector found flip top cooler and several other cooler door gaskets were not clean. All the lower shelving was not clean. Flass door cooler lower shelving and door tracks were not clean. The Interior of the upright refrigerator was not clean with food soil and mildew. Some hibachi carts were not clean with food soil. The bottom shelving of bar coolers was found with old adhering cardboard.
This violation was found six times.
Improper sanitary facilities and controls: The inspector found the faucet in the men’s bathroom was leaking around the faucet when used.
Improper sanitary facilities and controls: The inspector found the men’s bathroom and sinks in the kitchen were missing hand wash signs. This was corrected.
Inadequate insect/rodent control: The inspector found small flies in an unclean bar beverage gun boot.
Inadequate insect/rodent control: The inspector found the back door to the kitchen was propped open with a screened door. This was a possible entrance for flies and rodents.
Improper construction and maintenance of physical facilities: The inspector found some floor areas were not clean, primarily under and behind equipment and racks.
Kabul Restaurant
This restaurant had 11 violations, with none in critical condition.
Food not protected in general: The inspector found multiple bags of onions stored on the floor in the rear preparation area were not protected. Containers of water and soda products stored on the floor in the preparation area aisle were not protected either.
Poor sanitary design, construction, installation of equipment and utensils : The inspector found cutting boards with deep grooves and heavy staining.
Improper cleaning, washing and sanitizing of equipment and utensils: The inspector found cook line equipment throughout the facility not clean. All refrigeration equipment was not clean either. Cutting boards, the three-bay sink, bulk storage containers, and the door tracks of the glass door refrigeration equipment were found not clean.
This violation was found five times.
Improper sanitary facilities and controls: The inspector found the hand wash sink in the back area was slowly draining. The faucet system at the three-bay sink was found leaking, and the interior of the three-bay sink wasn’t clean and not maintained.
This violation was found two times.
Improper sanitary facilities and controls: The inspector found no soap or paper towels at the hand wash sink.
