    Ashe County health inspections

    The following information was provided by Appalachian Health Care. Restaurants and food stands are inspected one to four times per year, based on risk frequency, and institutional food service facilities are inspected four times per year.

    Dollar Mart

    209 South Main Street, Jefferson

    Grade: A

    Score: 94.5

    The store received a 1.5-point demerit for improper cold holding temperatures. Raw chicken, slaw, bologna, and chili were present in the walk in cooler ranging in temperature between 42 degrees and 47 degrees. Walk-in cooler ambient air registered 38 degrees after approximately 10 minutes. It is advised that walk-in refrigerator be serviced. Ambient air temperature may not be cold enough to maintain or cool foods to temperatures of less than 41 degrees when doors are frequently opened. The store received a 1.5-point demerit for Time as a Public Health Control procedures and records. Time was not written on the hot holding cabinet at the onset of the inspection, in addition to time missing from sandwiches. TPHC must be done correctly or the hot holding unit must be replaced and will be expected to keep food at least 135 degrees. Items ranged between 116 degrees and 139 degrees. Be sure to separate batches of food in order to keep up with time. Range of temperatures indicates a possibility of different batches in same pans. All food in hot holding unit must be discarded within four hours regardless of when the latest batch was cooked. Time was written on the unit during the inspection. The store received a 1-point demerit for plumbing repairs needed. Pipes under sink leak. Prep sink faucet leaks. Call plumber for repair. The store received a half-point demerit for utensils, equipment and linens improperly stored, dried and handled. Shelving where clean dishes and utensils are stored, have buildup of grease and dust and are very difficult to clean. Recommend replacing shelving. The store received a half-point demerit for equipment, food and non-food contact surfaces needing to be better cleaned/maintained. Handle is completely broken off hot holding cabinet. There are jagged edges that could cut hands and are also a cleanability issue. Shelving in walk-in cooler is rusting. Wooden shelving where food and linens are stored is peeling and needs repainting. A pair of tongs was stored at clean dish shelving in poor condition, and with deteriorating rubber. Person In Charge (PIC) was advised to discard tongs, as they are a physical hazard. Recommend removal and replacement of all damaged equipment. The store received a half-point demerit for physical facilities needing to be better cleaned/maintained.

    Dollar Mart received scores of 94.5 and 95 on two previous inspections.

    Kitchen Table Games & Bistro

    926 U.S. Highway 221 Business, West Jefferson

    Score: 97

    The restaurant received a 1.5-point demerit for improper cold holding temperatures. Prep cooler was not working properly during the inspection and was holding TCS foods at 43-44 degrees. Cut tomato was 44 degrees, garlic butter 43 degrees, chili (overstacked) 50 degrees, chickpeas 44 degrees, and honey curry chicken salad 44 degrees. Corrected by instructing PIC to discard chili and adjust temperature during inspection. The restaurant received a 1.5-point demerit for improper date marking. Open container of feta in bottom of prep cooler was dated March 23, sausage April 12, and another container of sausage April 26, chickpeas dated April 23. Hardboiled eggs in two-door cooler were dated as prepared April 23, and pizza sauce April 12. Datemark TCS foods, counting day one as the date of opening or preparing, and discard by the end of Day 7. Corrected by instructing PIC to discard each of these. Several items were not datemarked during inspection: open container of Greek yogurt, container of feta, open carton of coconut milk, and open bag of deli ham. Corrected by instructing PIC to discard if dates are not known. Recommended discard of open Greek yogurt since manufacturer use by date was April 23. Reminder that if items are cooked or prepared, then frozen on the same day, they should be datemarked when frozen, then mark through datemark when pulled from freezer for thawing.

    River House Restaurant

    1896 Old Field Creek Road, Grassy Creek

    Score: 95.5

    The restaurant received a 1.5-point demerit for improper cooling time and temperatures. Potato leek soup, cooked the previous day, registered between 42 degrees and 44 degrees in the walk-in cooler. Soup is in a densely filled, large container, and may not have cooled properly due to incorrect cooling methods. Walk-in cooler is holding foods between 40 degrees and 41 degrees, and has an ambient air temperature of 38 degrees. Separate large batches of food and use ice, ice paddle, or freezer to cool foods before placing into walk in cooler. It is recommended that walk-in get serviced before summer time heat plays a role in raising ambient temperatures of walk-in. The restaurant received a 1.5-point demerit for improper cold holding temperatures. A container of raw eggs were left out on the prep table, and registered 63 degrees. Egg was discarded. Do not leave raw, non-pasteurized eggs out at room temperature. Heavy whipping cream and half and half registered 51 degrees — 58 degrees in prep cooler, and may have been left out too long during breakfast. Do not leave TCS food out for long periods of time. TCS foods should be kept 41 degrees or less. Raw chicken in walk-in cooler registered 43 degrees. The restaurant received a half-point demerit for improper eating, tasting, drinking or tobacco use. Employee was eating and drinking at clean dish area while engaged in dish washing/storing duties. Eat in designated areas, and keep drinks underneath and away from clean dishes and food/food service items. A bottle of water was also present on the clean dish air drying table. Do not use bottles that have twist off lids to avoid contamination from mouth and hands. The restaurant received a half-point demerit for equipment, food and non-food contact surfaces needing to be better cleaned/maintained. Three compartment sink vat is cracked at the seam where it meets the drainboard. Rust is present on shelving in three-door cooler in kitchen. Chipping paint present on shelving and cabinetry, throughout. Butcher block counter top is heavily scored. Countertop around prep sink is damaged. Cutting boards and utensils observed in kitchen that are worn. Ice machine is leaking water onto floor. The restaurant received a half-point demerit for non-food contact surfaces needing to be better cleaned. Non-food contact surfaces such as the food processor need to be wiped down to remove food residue. Orange pulp present in clean dish area where orange juice is squeezed. Walk-in cooler floors, walls, and shelving are present with food crumbs and residues and need more frequent cleaning. Freezers are in need of defrosting and cleaning. Cabinetry and drawers need to be cleaned.

    River House Restaurant received scores of 95.5 and 96.5 on two previous inspections.

    Whistle Stop Cafe

    4969 NC Highway 88, Warrensville

    Grade: A

    Score: 98.5

    The restaurant received a 1.5-point demerit for improper cold holding temperatures. Raw hot dogs and raw pork chops were present in the prep cooler reach in registering 44 degrees. TCS foods must be held at temperature 41 degrees or less. Items were moved to an alternative unit. Prep cooler needs to be serviced.

    Whistle Stop Cafe received scores of 97 and 99.5 on two previous inspections.
