Birth Certificates
Cost of Copy: $5.00 for the first copy; $4.00 for each additional copy requested on the same application. Registrants over 60 years of age and Veterans of the United States Armed Forces can obtain copies free of charge (maximum three copies per registrant per year).

Remarks: The Central Office of the Demographic Registry holds records for all citizens born on or after June 22, 1931. Earlier records can be obtained from the Local Registrar’s Office in the municipality where the event occurred. Payments must be made via money orders payable to the Secretary of Treasury. Neither cash nor personal/bank checks are accepted. For current fees, call 787-765-2929 Ext. 6131. Applications must include a legible photocopy of a valid ID (driver’s license, passport, or government-issued ID). Veterans must submit a copy of their DD214 or Veterans Affairs ID card to receive benefits. Applicants must be an interested party as defined by law.
Death Certificates
Cost of Copy: $5.00 for the first copy; $4.00 for each additional copy requested on the same application. Beneficiaries of a Veteran of the United States Armed Forces can obtain copies free of charge (widow or children under 21 years of age).

Remarks: The Central Office holds records for divorces registered on or after 1941. Divorce certifications are not actual divorce decrees but abstracts of the final court resolution. Availability varies; not all decrees are submitted to the Demographic Registry. If a decree is not registered, a negative certification will be issued for the same cost. Payments must be made via money orders payable to the Secretary of Treasury. Neither cash nor personal/bank checks are accepted. For current fees, call 787-765-2929 Ext. 6131. Applications must include a legible photocopy of a valid ID. For court-issued divorce decrees, contact the Superior Court where the divorce was granted.
