CHILLICOTHE — A consent agreement from the State of Ohio Counselor, Social Worker, Marriage and Family Therapist Board has revealed that local therapist Alvin Mares recently had his license revoked and suspended.

Mares previously worked at Mares Cares, a counseling center with multiple locations in the Chillicothe area. After this action, he is no longer listed as a provider on the website.

The agreement states that Mares was an independent social worker who received his license in 2016. During 2022-2024 the agreement states that Mares was failing to keep proper records, failed to avoid a multiple relationship situation with a client and failed to maintain client confidentiality.

With this information, Mares's supervision designation of his independent social worker license was revoked and his license to practice as an independent social worker was suspended for one year. After the suspension, if Mares chooses to re-enter the field all aspects of his social work practice must be monitored and he is to receive face-to-face monitored supervision of at least one hour every week for two years. He is also required to complete six hours of continuing ethics education with three hours being related to boundaries at his expense, according to the agreement.

The agreement shows that Mares read and understood the agreement which took effect on July 18.
