Tom Selleck is undergoing physical therapy, but he’s still in bad shape and struggles to walk, sources exclusively tell Closer.

“He has those scars on his knees, which would indicate he’s had surgery on them recently. But clearly, it’s not going so well and taking a long time to heal,” says an insider of the 79-year-old actor, who was recently spotted out and about in the San Fernando Valley looking particularly glum.

Earlier this year, Tom was captured on video obtained by In Touch in which he appeared to have large vertical scars on both of his knees. As seen in the clip, Tom visibly had trouble walking when leaving the gym and stopped in front of his truck and held onto the hood of his car.

“He’s got specialists helping, and he’s doing his rehabilitation at the gym where he putters around, and it’s slow going,” Closer‘s source explains. “The fact remains he’s unsteady on his feet and has trouble walking unassisted.”

While it’s hopeful that Tom will get better by making changes to his lifestyle, “If things don’t improve, he’ll have to swallow his pride and use a walker before long,” the source explains.

“At least he’s doing the right thing and having physical therapy, but it’s all taking a toll as he closes in at 80,” the insider continues. “What he really needs to do is lose weight because all those extra pounds aren’t doing his joints any good.”

A separate source told Closer in July that Tom has been eating comfort food to make up for the termination of his beloved show Blue Bloods.

Medical experts and sources estimated that the 6-foot-4 actor is grinding the scales at 350 pounds, which is a 150-pound gain from his 1980s heyday on Magnum, P.I.

“He’s bigger than before, and it’s a shame to see him this way, so heavy and struggling to squeeze behind the wheel of his car,” said an insider.

It was a sad day for Tom and Blue Bloods fans to see the show get the boot after its 14th season wrapped in June, so “it breaks his heart the show’s over,” an insider explained. “These people were family to him, and they went out for lots of fancy meals toward the end. He’s been continuing that trend, judging by the looks of him!”

Sources said that although Tom vows to hit the gym, loved ones fear he’s putting his health at risk with his diet.

“He needs to shed pounds,” said an insider. “Anyone can see that, but it’s not going to be easy. Clearly, this forced retirement hasn’t done him much good!”

Earlier this month, Tom addressed the end of Blue Bloods, telling TV Insider, “I’m kind of frustrated.”

“So how do I feel? It’s going to take a long time to sort all of this out,” the Emmy winner confessed. “I remember after the weekend [of the final episode’s shoot], I said, ‘I’ve got to get to bed early tonight because I have to do my dialogue for Monday.’ Well, there was no Monday. It’s just going to take a while.”
