In Episode 7 of Grotesquerie , the twist reveals that the entire story up to that point was a manifestation of a comatose Lois Tryon’s mind.

Ryan Murphy told TheWrap , “It was always baked into the DNA of Grotesquerie,” indicating that this was planned from the start. John Landgraf, FX Networks chairman, said, “It hasn’t fully established itself as the series it wants to be until the end of the 10th episode.”

Murphy explained how details like the burning curtain in the opening scene referenced Lois’ hospital bed. The series includes subtle nods to Lois’ coma. For example, Lesley Manville’s character eats cherries, symbolizing Lois’ subconscious fear of blood clots. Murphy noted, “Every scene in [their meeting] was designed to feel like a dream.” This includes the idyllic encounter with Eddie, played by Travis Kelce.
