    ABOUT RESTAURANT INSPECTIONS

    This is a snapshot of the inspections conducted by the Polk County health department.

    Twice annually, licensed restaurants receive unannounced inspections that focus on food temperatures, food preparation practices, worker hygiene, dish-washing and sanitizing, and equipment and facility cleanliness.

    Violations: Restaurant scores are based on a 100-point scale. Priority violations deduct 5 points, and priority foundation violations deduct 3 points. Violations recorded on consecutive inspections result in point deductions being doubled.

    Scoring: Scores of 70 or higher are considered compliant. Restaurants scoring below 70 must be reinspected within 30 days or face closure or other administrative action. Restaurants display a placard by the entrance to indicate whether they have passed their last inspection.

    Medicine is improperly stored or labeled, specifically: Pill organizer of unlabeled medications is on shelf next to prep unit. Shelves underneath are used to store utensils, dishes and food products. Point deduction: 5.

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Raw salmon sitting on ice in metal container is at 55 degrees Fahrenheit. Point deduction: 5.

    Priority violations :

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Several food items in reach-in fridge are at 45 degrees Fahrenheit (strawberries and cheesecake). Point deduction: 5.

    Golden Hop Saloon

    Location: 133 C St., Independence

    Date: May 3

    Priority violations :

    Hand towels or a hand drying device is not provided at the handwashing sink, specifically: Handwashing sink in kitchen is out of paper towels. Point deduction: 3.

    The chlorine sanitizer concentration, pH, or temperature is not adequate, specifically: Dishwasher is not dispensing sanitizer - 0 parts per million. Point deduction: 5.

    Potentially hazardous food is improperly reheated for hot holding, specifically: Nacho cheese, gravy and soup in steam table are between 109 and 130 degrees Fahrenheit. Operator states these were moved from the refrigerator to the steam table almost two hours ago. Point deduction: 5.

    Raw or ready-to eat food is not properly protected from cross contamination, specifically: Container of raw eggs is sitting on packages of ready to eat prosciutto and cheese in white reach-in by stove. Point deduction: 5.

    Food-contact surfaces are not clean, specifically: Cutting boards on prep unit are not clean. Grooves from knives have an accumulation of food buildup. Point deduction: 3.

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Container of butter with chopped fresh garlic, container of oil with fresh garlic and pan of lasagna (64 degrees Fahrenheit) are sitting out at room temperature. Operator states these have been out for 20 minutes. Drawers under stove are not holding food cold enough. Several bags of cooked noodles and chopped lettuce are 47 degrees Fahrenheit. Operator states the items were put in the unit one to two days ago. Point deduction: 5.

    Refrigerated, ready-to-eat, potentially hazardous food has not been consumed within the required time period or is not properly date-marked, specifically: Pan of cooked noodles in white fridge is datemarked April 25, 12 days old. Point deduction: 5.

    Temperature measuring devices are not provided or readily accessible for checking food temperatures, specifically: No probe thermometer on-site. Point deduction: 3.

    Paet Riew Thai Kitchen

    Location: 180 Main St. E, Monmouth

    Date: May 10

    Score: 85

    Priority violations :

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Soup on stove is hotholding at 118 degrees Fahrenheit. Operator states this was cooked about three hours ago. Point deduction: 5.

    Poisonous or toxic materials are not properly separated or located, specifically: Pack of cigarettes and container of petroleum jelly are sitting on shelf above prep unit and containers of vinegar and sauces. Point deduction: 5.

    Westside Taphouse and Growler Fill

    Location: 1594 Edgewater St. NW, Suite 110, Salem

    Date: May 13

    Score: 100

    The Salty Shack

    Location: 1233 Riverbend Road NW, Salem (mobile unit)

    Raw or ready-to eat food is not properly protected from cross contamination, specifically: Container of raw chicken is on top shelf of prep unit over container of cooked bacon. Point deduction: 5.

    Potentially hazardous food is not maintained at proper hot or cold holding temperatures, specifically: Metal container of liquid raw eggs is sitting out (49 degrees Fahrenheit). Operator states it has been out for 20 minutes. Plastic shopping bag of rice is sitting in microwave at 102 degrees. Operator states it has been out of temperature control for under four hours. Point deduction: 5.
