Income Verification

Custody Documentation (if applicant is NOT on birth certificates)

Verification of Governmental Assistance (if applicant receives SNAP, Medicaid, TANIF) *NO CHILDREN OVER THE AGE OF 1 ARE ALLOWED AT REGISTRATION*
