    Violations found at these Bartlesville restaurants inspected in August and September

    The Oklahoma State Department of Health conducts licensing and inspection of food establishments.

    Below are the results of the August and September inspections in Bartlesville, based on results reported in the department's online database .

    Bartlesville restaurants and establishments with food inspection violations

    The following restaurants reportedly had violations in the last series of inspections:

    • Dollar General #4546 1501 SW Frank Phillips Blvd (Inspected 9/3/2024): One violation was found concerning the cleanliness of non-food-contact surfaces.
    • Family Dollar #4884 (Inspected 8/27/2024): One violation was found concerning food separation and protection.
    • For Your Convenience (Inspected 8/27/2024): One violation was found concerning pest control issues.
    • IHOP #3532 (Inspected 8/27/2024): Two violations were found concerning proper food temperatures and cleanliness of food surfaces and utensils.
    • Monterey's Little Mexico (Inspected 9/3/2024) : Six violations were found concerning pest control, cleanliness of food surfaces and utensils, eating, drinking, and tobacco use, discharge from eyes and nose, cleanability food and non-food contact surfaces and maintenance of physical property.
    • Ollies Bargain Outlet (Inspected 8/26/2024): One violation was found concerning food labeling.
    • Pho Than Vietnamese Noodles & Restaurant (Inspected 9/18/2024): Health inspectors found 16 violations concerning the person in charge being present and knowing proper required knowledge, hand cleanliness; bare hands used on ready-to-eat foods; hand washing facilities; food separation and protection, pest control issues, dishware washing sanitation issues, using approved food thawing methods, eating, drinking, and tobacco use, discharge from eyes, nose, proper storage and cleaning of utensils, single-use articles properly stored and used, cleanliness of food and non-food contact surfaces and maintenance of physical property.
    • Tri County Technology Center (Inspected 9/17/2024): Five violations were found concerning the cleanliness of food surfaces and utensils, eating, drinking, and tobacco use, discharge from eyes and nose, cleanliness of non-food surfaces, hand wash facilities and cleanliness of premises.
    • Walmart #41 (Inspected 8/27/2027): Seven violations were found concerning hand wash facilities, pest control issues, dishware washing sanitation issues, cleanliness of food surfaces and utensils, cleanliness of food and non-food contact surfaces, plumbing maintenance issues and maintenance of physical property.

    The following food service locations were operating without a valid up-to-date permit or license in August and September, according to the state Department of Health database.

    The following Bartlesville food service locations were inspected in August and September, and no violations were found, according to the state Department of Health database.
