FORT DRUM, N.Y. (WWTI) – Fort Drum Regional Health Planning Organization and North Country Health Compass Partners are looking for nominations for this year’s Community Health Heroes.

The organizations are seeking “anyone who has demonstrated outstanding public service and a commitment to improving the health and wellness of their community can be nominated, and one individual from each county will be chosen to receive a 2024 Community Health Hero Award.”

Three area residents honored at Health Hero luncheon

The nominees must fall under the following criteria:

    • Live or work in Jefferson, Lewis, or St. Lawrence counties (not necessarily in the healthcare field)
    • Dedicate time and resources toward the improvement of community health and wellness
    • Show a level of care and compassion that exceeds that which is demonstrated by an average citizen

Community members may submit one nomination for consideration. Volunteers from the North Country Health Compass Partners will then pick one recipient from each of the counties (three awards total) based on the following factors: relation to health and wellness; scope of impact; depth of impact; social influence; volunteerism; and uniqueness. Winners will be honored locally during the Community Health Heroes Luncheon on Thursday, November 7, and throughout New York State on November 21, which is National Rural Health Day.

We are asking community members from the three-county region to nominate one individual that they believe embodies what it means to be a ‘Community Health Hero.’ This could be a healthcare professional, a volunteer, a business owner, an advocate, or anyone else that has actively worked towards improving the health and wellness of our region.

The nomination form can be found by visiting www.fdrhpo.org or by clicking this LINK. All nominations must be submitted by 4 p.m. August 30.
