    Crawling insects in kitchens, rodents and more in Upstate restaurants with worst June inspections

    South Carolina health inspectors found multiple violations at restaurants in the Upstate in June, from insects crawling on kitchen cabinets to rodent activity and rotting food. The state each month releases grades and inspection reports for food establishments across Upstate counties.

    Here are the restaurants that received some of the lowest ratings in June.

    Armando’s Burritos at 206-A Pine Knoll Drive in Greenville

    Armando’s Burritos had an inspection on June 4 and got a C grade.

    An inspector observed multiple cans of food rusted and with severe dents.

    Certain foods were seen not allowed to cool properly. Multiple food items were seen in the cooler without proper date markings.

    The restaurant had a required followup inspection on June 14 and got an A grade.

    Super Mercado Latino at 1605 Bridge Road in Greenville

    Super Mercado Latino had an inspection on June 12 and got a C grade.

    An inspector observed crates of tortillas stored on the ground.

    A buildup of debris was seen on the meat saw. A buildup of debris was also seen on the tortillas machine.

    Certain foods were seen not kept at proper holding temperatures. Chemicals were seen stored on shelves with food products.

    The restaurant had a required followup inspection on June 21 and got an A grade.

    Tokyo Grill and Supreme Buffet at 6124 White Horse Road in Greenville

    Tokyo Grill had an inspection on June 12 and got a C grade.

    An inspector observed signs of rodents in the dry storage room. Also observed was the gaps and a broken screen in the restaurant’s back door.

    A buildup of food debris was seen on shelving and the hood ventilation system. The walk-in cooler was seen unable to keep foods at proper temperatures.

    A buildup of debris was seen in the ice machine. Knives were seen stored between the prep cooler and table with visible debris on the surfaces. Employees were seen handling dirty dishes, then handle clean dishes without washing hands.

    The restaurant had a required followup inspection on June 28 and got an A grade.

    Waffle House #1725 at 8006 White Horse Road in Greenville

    Waffle House had an inspection on June 18 and got a C grade.

    An inspector observed crawling insects under a sink and in cabinets in the front kitchen.

    A buildup of debris was seen on the waffle makers. A buildup of debris was seen on the potato slicer and the mixer. The dish machine was seen not reaching the required temperature for sanitation. A buildup of debris was seen in the prep cooler and on shelving.

    The restaurant had a required followup inspection on June 21 and got an A grade.

    Mustard Seed BBQ at 2000 S J Workman Hwy in Spartanburg

    Mustard Seed BBQ had an inspection on June 18 and got a C grade.

    An inspector observed that the cook did not wash hands in between handling raw foods and ready-to-eat foods.

    Dried food residue was seen on shelves and the interior walls of the reach-in refrigerator. Certain foods were seen not kept at proper holding temperatures. An accumulation of grease was seen on kitchen walls, on the floor underneath equipment and on storage shelves.

    The restaurant had a required followup inspection on June 26 and got an A grade.

    Country Cook-In at 27 East Main Street in Greenwood

    Country Cook-In had an inspection on June 6 and got a C grade.

    An inspector observed rodent droppings under shelving in a small storage closet.

    Employees were seen not washing hands or change gloves. Food products were seen with signs of decay, with white and green substances on them.

    Certain foods were seen not kept at proper holding temperatures. Multiple seafood products were seen thawing at room temperature in a bucket. An accumulation of grease, grime and food residue was seen on floors and walls in the facility.

    The restaurant had a required followup inspection on June 12 and got an A grade.

    Dozo Sushi and Hibachi at 6606 Calhoun Memorial Hwy in Pickens

    Dozo Sushi and Hibachi had an inspection on June 26 and got a C grade.

    An inspector observed several rat droppings and live roach activity throughout the facility.

    Food contact equipment was seen as dirty due to construction on the building from a gas leak. Certain foods were seen not kept at proper holding temperatures. Food was seen stored on the floor of the walk-in cooler. A wall behind the cook line was seen damaged from a vehicle that had hit it.

    The restaurant had a required followup inspection on July 10 and got a B grade. A second required followup inspection has not yet been held.
