    AUG. 15

    El Amanecer Latino Market, 4806 Nine Mile Road – The following violations were reported during a routine inspection: neither kitchen hand sink has handwash signage posted; raw pork, beef and chicken are stored in separate plastic bags but in the same container in walk-in cooler; raw beef stored over produce in walk-in cooler; tamales in walk-in cooler are stored uncovered; dried food debris found in both microwaves; several spray cleaner bottles are unlabeled; observed flies and several roaches in kitchen; containers of lettuce and flour are stored directly on kitchen floor; multiple food items stored directly on walk-in cooler floor; cutting boards are scratched and stained; three-door reach-in freezer door gaskets are torn; deep fryer baskets are in disrepair; there are two lights out in the hood system; reach-in cooler door gaskets throughout kitchen are dirty; observed grease residue on sides of hot line cooking equipment; walk-in cooler fan guards are dusty; trash can in women’s restroom is uncovered; there are some cracked floor tiles in front of kitchen back door.

    El Rey International Supermarket, 6006 West Broad Street – The following violations were reported during a routine inspection: no one on staff has food protection manager certification; no handwash signage is posted at bar area hand sink; raw fish stored over ready-to-eat foods in walk-in cooler; no date marks found on multiple prepared foods; various medications and first-aid supplies are stored next to food in kitchen; whole cuts of meat are thawing at room temperature in three-compartment sink; bulk containers of white powders are unlabeled; multiple boxes of food are stored directly on walk-in freezer floor; reach-in cooler and reach-in freezer have torn gaskets; wire racks throughout kitchen are rusty; hot line reach-in cooler door is in disrepair; cutting boards are scratched and stained; facility doesn’t have sanitizer test strips on site; cooler door gaskets throughout kitchen are dirty; observed grease and food debris on hot line reach-in cooler interior and racks; there is dried animal blood in bottom of reach-in freezer; observed grease buildup on sides of hot line equipment; there are cracked and missing floor tiles in front of hot line; floor under hot line, wall behind hot line and floor under three-compartment sink need cleaning.

    AUG. 16

    Lulabelle’s Café & Catering, 2012 Staples Mill Road – The following violations were reported during a routine inspection: no one on staff has food protection manager certification; mold found inside ice machine; no date marks found on chicken/tuna salad made yesterday; two reach-in coolers in kitchen are in disrepair (staff is only using tops of units); cutting boards throughout kitchen are scratched and stained; there is grease buildup on sides of hot line cooking equipment; door gaskets on reach-in cooler door and chest freezer door are dirty; observed dried food debris on shelves under hot line and prep table with microwave.

    AUG. 19

    Domino’s Pizza, 5526 Lakeside Avenue – The following violations were reported during a routine inspection: an employee handwash policy isn’t posted in the employee bathroom; observed an abundance of small flies around mop sink and three-compartment sink within the back washroom; pull out drawer on make table unit and cover for refrigeration unit are broken; there is dust buildup behind and above oven unit; observed food debris and buildup on computer keyboards; area behind units is dusty; there is a leak on the bottom of the hand wash sink next to prep table; ceiling tiles next to oven are dirty and stained.

    Taco Bell, 11450 West Broad Street – The following violations were reported during a routine inspection: temperature of container of precut tomatoes is elevated; an employee with long chin hair isn’t wearing a beard guard; gasket is broken on kitchen reach-in freezer door; interiors of warmers are dirty; mop sink faucet doesn’t have a dual check valve installed; walk-in freezer floor is dirty.

    Coco Café & Bistro LLC, 8703-A West Broad Street – The following violations were reported during a routine inspection: facility doesn’t have a certified food protection manager; raw meat stored over cooked shredded pork and chicha in reach-in cooler; temperature of chicha made yesterday in reach-in cooler is elevated; temperatures of milk and yogurt in front prep unit are elevated.

    Li’s Garden Inc., 1237 North Laburnum Avenue – The following violations were reported during a routine inspection: raw shrimp stored over cooked chicken in walk-in cooler; observed food debris inside and outside meat grinder, on knives hanging on clean knife rack, bulk food containers stored clean, and can opener blade; menu states that scallops and crab rangoon are being served but facility is serving imitation scallops and imitation crabmeat (menu needs to reflect this); food is stored directly on walk-in cooler floor; single-use egg crates are being reused throughout the kitchen; plastic is being cleaned and reused; cutting boards throughout the kitchen are scratched and stained; deep fryer baskets are in disrepair; to-go plastic bags are being used to store raw food; observed grease buildup on sides of hot line cooking equipment; reach-in cooler door gaskets throughout kitchen and walk-in cooler door gaskets are dirty; observed dried food debris under dry storage shelves, on side of hot line reach-in cooler and shelf under prep table; there is grease and grease buildup on floors under hot line equipment.

    AUG. 20

    Little Caesars, 9514 Woodman Road – The following violations were reported during a routine inspection: paper towel dispenser at hand wash sink next to drive-thru is broken; an employee is wearing bracelets and jewelry on wrist; observed food debris and grease buildup on pizza staging racks, dough racks in walk-in cooler and storage rack next to three-compartment sink; observed dust buildup on fan blades within walk-in, above hot holding units and above oven; there isn’t an air gap between the water supply outlet and the flood rim level of the three-compartment sink; trash can in unisex employee bathroom is uncovered; there are cracked and/or uneven floor tiles next to pizza oven and three-compartment sink; floor wall juncture underneath three-compartment sink is not coved and closed to no larger than 1/32-inch space; there are food splatters on wall behind dough maker; ceiling tiles near pizza oven are stained; floor under storage rack next to three-compartment sink is dirty.

    Brazil Tropical Steakhouse, 9035 Quioccasin Road – The following violations were reported during a routine inspection: hand sink in BBQ room doesn’t have paper towels; bar hand sink doesn’t have hand soap; there is 0 ppm chlorine at three-compartment sink in main kitchen; temperature of fried tilapia at buffet line is too low; temperature of milk in thermos is elevated; temperatures of pasta salad, coleslaw and salad at buffet line are elevated; temperature of sliced deli meat in mini fridge at coffee bar is elevated; sliced deli meat in mini fridge is past its expiration date; white powders on food storage shelf are unlabeled.

    Five Guys, 12210 West Broad Street – No violations reported during a routine inspection.

    White Tiger Indian Restaurant & Bar, 3991-K Glenside Drive – The following violations were reported during a routine inspection: tub of raw chicken is stored over boxes of lamb chops; dried food debris found on immersion blender in kitchen; tilapia is thawing in vacuum sealed packaging; cases of tomatoes and oil jugs are stored directly on kitchen floor; a knife is stored in a gap between prep tables; in-use utensils are stored in ambient temperature standing water; seal between three-compartment sink and wall has worn away; food items are stored in non-food grade plastic to-go bags; observed grease buildup on sides of deep fryer; door gaskets of reach-in cooler on hot line are dirty/moldy; outdoor dumpster doors were left open; floor below and wall behind deep fryer are greasy; wall behind three-compartment sink is mildewy; hood vents and fire suppression nozzles have grease residue forming/dripping.

    AUG. 21

    Burger King, 5217 Williamsburg Road – The following violations were reported during a routine inspection: paper towel dispenser at hand sink next to three-compartment sink is broken; there isn’t a time control sticker posted on sliced tomatoes tray on prep line; boxes of fries and cookie stored directly on walk-in freezer floor; there is heavily soiled cardboard underneath and next to fryers; several items are broken (ice cream machine, hinges on sliding freezer cover and fryer baskets); there is dust and grease buildup on monitor cables; observed grease buildup around fryers, rear storage rack and on broiler rack; observed leak in sanitizer dispenser at mop sink when under pressure; ceiling tiles are dirty and stained; floors under three-compartment sink, prep sink and prep line need cleaning; there is substantial buildup of grease and food debris on fryer hood system and person in charge couldn’t indicate when hood was last serviced.

    McDonald’s, 4708 Williamsburg Road – The following violations were reported during a routine inspection: temperature of sausage burritos prepared yesterday is elevated because they were not cooled properly (wrapped and covered in a pan); multiple employees are wearing bracelets or jewelry on wrist while conducting food service operations; frappe machine in drive-thru prep area is broken; sheet pans are dented and worn; fryer baskets are broken; observed dust buildup above coffee makers/frappe station; observed food debris buildup in single-service cup storage; there is water pooling near the soda and ice storage area; floor tiles next to drive-thru area are dirty; observed grease buildup on floor beneath fryers and grills.

    McDonald’s, 2310 Mechanicsville Turnpike – The following violations were reported during a routine inspection: soap dispenser in men’s restroom isn’t working; no handwash signage is posted in men’s restroom; fry station hand sink doesn’t have paper towels; dried food debris found inside microwave; spray bottle of degreaser is stored next to cooked bacon; ice scoop is stored with the handle touching ice; outdoor dumpster doors were left open and a lid is missing; observed damaged floor coving tile under three-compartment sink and next to reach-in cooler door.

    AUG. 22

    DQ Grill & Chill, 3275 New Market Road – The following violations were reported during a routine inspection: temperature of hot dogs in lowboy reach-in unit is elevated; this unit is not holding food at the proper temperature; gaskets on make table are torn; storage racks in walk-in fridge are rusty; sliding freezer drawer is broken; small cutting board in ice cream prep area and large cutting board are scratched and discolored; there is no air gap present at three-compartment sink; observed dust buildup on air vent by three-compartment sink and fan blades within walk-in; there is debris around the mop sink; lighting on hood is not operational.

    Subway inside Fas Mart, 2650 New Market Road – The following violations were reported during a routine inspection: no certified food protection manager is on site; cutting boards on prep line, make table and in dry storage are scratched and stained; no indirect connection present on three-compartment sink or prep sink; observed dust and dirt under storage racks next to three-compartment sink.
