    JULY 24

    Pho Que Huong 1 LLC, 10073 Brook Road – The following violations were reported during a routine inspection: bar hand sink doesn’t have a handwash sign posted; raw shell eggs stored over ready-to-eat foods in reach-in cooler; raw meats stored over beef stew in walk-in cooler; dried food debris and rust found on can opener blade; observed mold inside ice machine and on exterior sliding doors/lid; temperature of white rice in rice cooker is too low; no date marks found on prepared items (cooked meats, pho broth and soups); the disclosure statement on the menu doesn’t identify raw or undercooked foods with asterisks; more freezer space is needed as the walk-in freezer is stored so tightly with food from floor to ceiling that it’s hard to add or remove any items; food is stored directly on walk-in freezer floor; back-up reach-in cooler has a torn door gasket; one light is out in hood system on back hot line; paper towel dispenser in dish area is in disrepair; cutting boards are scratched and stained; observed heavy grease buildup on sides of hot line equipment including space between stove and grill; reach-in cooler door gaskets throughout kitchen are dirty and moldy; there is significant grease buildup on hood filters.

    Thai Flavor Food & Noodle, 4024-D Cox Road – The following violations were reported during a routine inspection: facility couldn’t provide written procedures for cleaning up vomit or diarrhea; kitchen hand sink is blocked by a cart; ice machine in kitchen is dirty; temperatures of sprouts and noodles are elevated; no date marks found on prepared food items; bottle with orange liquid is unlabeled; frozen packaged meat is stored directly on walk-in freezer floor; small cutting boards and boards on kitchen line are scratched and stained; storage shelves are lined with garbage bags and plastic wrap; sanitizer test strips are damaged and missing their expiration date; several items/areas are dirty (walk-in refrigerator racks and fan guards, walk-in freezer racks, fan guards and floors, drying racks above three-compartment sink, and top cooking equipment/utensil shelf); a light bulb is out in walk-in freezer.

    Nineveh Bakery & Special Food, 8026-C West Broad Street – The following violations were reported during a routine inspection: both bathrooms are missing paper towels; temperature of fish that just came out of the oven is too low; temperatures of diced tomatoes and lettuce in prep unit are elevated; mop isn’t hung to air dry.

    JULY 25

    Hooters of Richmond, 7912 West Broad Street – The following violations were reported during a routine inspection: cook on the prep line is chewing gum; sanitizer level is at 0 ppm at bar dish machine (container is empty); temperatures of just-prepped cut tomatoes and cabbage in the prep unit are elevated; bulk spice ingredients are unlabeled; metal wires are broken and bent on fryer baskets and breading basket; walk-in freezer door is in disrepair; there is heavy ice buildup around walk-in freezer door; observed buildup of grease and food debris on surfaces of fryer wheels, storage racks in coolers, lexan pans storing to-go cups, under grill and on grease filter machine; mop sink doesn’t have the proper backflow prevention device installed; there is a leak under hand sink near cook line; observed food debris and grease buildup on floors in coolers, cook and prep lines, dry storage and bar floor drain; floor in mop room is not smooth and easily cleanable; there are broken floor tiles in walk-in cooler; bottom of walk-in cooler is rusted and in disrepair.

    Qdoba—Short Pump, 11282 West Broad Street – The following violations were reported during a routine inspection: paper towel dispenser by hand sink next to grill is empty; several items in the kitchen are rusty (two transport carts, walk-in refrigerator racks, dry ingredients storage racks and drying rack above three-compartment sink); ice machine lid is not functioning correctly (it’s catching condensation in the lip and pouring it into the interior of the machine); walk-in refrigerator fan guards and floor are dirty.

    Cantonese Kitchen, 9047 West Broad Street – The following violations were reported during a routine inspection: a strainer found in serve area hand sink; temperature of chicken at the fryer is too low; no date marks found on egg rolls or noodles in the lowboy; temperature of BBQ meats in display case is too low because of improper time control methods (person in charge states that BBQ meats are held at room temperature in the display case and reheated every two hours throughout the day); rice scoops found in ambient temperature standing water.

    Mi Guerrero Grill, 61 South Laburnum Avenue – The following violations were reported during a follow-up inspection: observed staff enter the kitchen area and begin handling food and utensils without washing hands first; staff are washing hands in three-compartment sink instead of hand sink; a hand sink in the kitchen is blocked by a prep table; food debris found on can opener blade; raw shell eggs stored on a shelf at room temperature; spray bottles used for cleaning are unlabeled; observed staff thawing chicken, shrimp and beef at room temperature; multiple cutting boards are heavily scratched and stained; coolers throughout the kitchen have torn door gaskets; rice is being stored in non-food safe plastic bags; facility doesn’t have sanitizer test strips; there isn’t a covered trash can in the women’s restroom; the incorrect ceiling tile is used in the food prep area; there are stained, damaged and missing ceiling tiles throughout the establishment; there is a hole in the wall behind the prep sink.

    JULY 26

    El Nopal II Inc., 5205 South Laburnum Avenue – The following violations were reported during a routine inspection: there is no certified food protection manager on staff; hand sink in kitchen doesn’t have hot water (valve is broken); raw shell eggs and raw chicken stored over raw beef in walk-in cooler; ice machine has a layer of pink slime buildup on the inside surface; mold found inside beverage area soda gun; bag of scallops is thawing at room temperature; there is no food thermometer in the kitchen; back door was left open and the screen door has holes big enough for pests to enter through; two buckets of food stored directly on walk-in cooler floor; wire racks in walk-in cooler are rusty and moldy; fry basket is in disrepair; cutting boards throughout kitchen are scratched and stained; coolers throughout kitchen have dirty door gaskets; outdoor dumpster lids were left open; there are food stains on walls throughout kitchen, especially in dry storage area; ceiling tiles throughout kitchen are stained and damaged; floor in boiler room has significant buildup of trash and debris; floors and wall near hot line are greasy; lights are not working in the hood system; there are cracked floor tiles in boiler room, near mop sink, in front of back door and in front of ice machine; there is a hole in the wall in the men’s restroom; observed grease buildup in hood system; grease is dripping from fire suppression system pipes and light fixtures and the exteriors are greasy and dusty.

    Ginza Spot, 3402 Pump Road – The following violations were reported during a routine inspection: facility doesn’t have written procedures for cleaning up vomit or diarrhea; temperatures of tomatoes and lettuce in front make table are elevated because they are double panned; spray bottle containing clear liquid is unlabeled; salmon fillets are defrosting in reduced oxygen packaging; frozen meat is thawing in standing water in three-compartment sink; several packaged food items are stored directly on walk-in freezer floor; take-out containers are not stored inverted on expo shelf; small cutting board is heavily scratched and long cutting boards are stained; dumpster area is surrounded by trash and loose debris; wall behind and drain under three-compartment sink are dirty; prep sink in front of facility is dirty; several mops are not hung to air dry; tile is broken at corner of walk-in freezer area.

    Starbucks Coffee Company, 6201 River Road – The following violations were reported during a routine inspection: observed buildup on panel inside ice machine; temperatures of sweet cream in drive-thru unit and whipped cream in unit near tea station are elevated; sanitizer solution for wipes is measuring 0 ppm; plastic bins are stacked while still wet; floors under counters and refrigeration units at front line are dirty.

    Anokha Indian Cuisine, 4015 Lauderdale Drive – The following violations were reported during a routine inspection: hand sink in kitchen is blocked by a ladder; hand sink behind bar doesn’t have paper towels; facility couldn’t provide parasite destruction documentation from salmon provider; ice machine is dirty; menu doesn’t contain asterisks linking to the consumer advisory for salmon and lamb that can be made to order; meat is thawing on top of freezer chest; boxed chicken is stored directly on walk-in refrigerator floor; take-out containers are not stored inverted on expo line; fryer basket is broken; drying rack above three-compartment sink is rusty; linen is lining several racks in kitchen; long cutting boards on prep lines are heavily scratched and stained; dumpster area is surrounded by trash and loose debris; observed mildew buildup on wall behind three-compartment sink; ceiling vents throughout kitchen are dusty.

    JULY 29

    Tiki Tiki, 8917 Patterson Avenue – The following violation was reported during a follow-up inspection: walk-in cooler door gasket is torn.

    Captain D’s Seafood, 4800 Williamsburg Road – The following violations were reported during a routine inspection: facility couldn’t provide employee health policy; person in charge rinsed used batter container and placed back in prep area without sanitizing; temperature of cole slaw in walk-in is elevated; breading mixtures sifted every two and four hours are not marked with the time; improper cooling methods observed for coleslaw and sliced tomatoes (stored in covered containers); observed multiple drain flies near kitchen hand sink; fly tape is stored above a food contact surface; single-service items are not stored inverted on cook line and in prep areas; food debris found on racks; grease and food debris found on kitchen equipment; walk-in fan guard is dusty; there is a leak under hand sink in back kitchen; water from ice machines is leaking onto the floor from a pipe; observed buildup of food grease outside of refuse areas; there is worn grout and water pooling within cracks near entrance to kitchen; ceiling tiles in kitchen prep area are damaged; floor tiles in kitchen are missing/cracked; observed buildup of grease on cook line floor.

    Clean Eatz Richmond, 11801 West Broad Street – The following violations were reported during a routine inspection: open drinking cup found next to prepping area; hand sink next to three-compartment sink doesn’t have paper towels; hand sink in back of kitchen is blocked by boxes; found a large, dented can of marinara sauce in storage; temperatures of breaded chicken and sweet potatoe fries in make table are elevated; food items are being thawed in standing water in prep sink; employee with beard who is working with food isn’t wearing a beard guard; take-out containers are not stored inverted; sanitizer test strips are expired; walk-in cooler fans and ceiling vents in kitchen are dusty.

    Taqueria y Restaurante El Chido LLC, 7568 West Broad Street – The following violations were reported during a routine inspection: hand sink at bar isn’t operational; bread and rice are stored in non-food grade plastic grocery bags; dried food debris found on can opener blade; concentration of chlorine sanitizer in three-compartment basin is too high; temperatures of steak, refried beans and chicken in prep unit are elevated; prep unit cutting boards and single cutting boards are heavily scratched and stained brown; gasket is torn on walk-in cooler door; walk-in cooler wire storage racks are rusty; light bulbs above prep unit and cookline are not shielded, coated or otherwise shatter-resistant.
