    • Observed: There was a bus tub partially full of cooked rice setting on the counter at room temperature, which was 74 degrees F. Voluntarily discarded by staff.
    • Observed: There were signs of mice in cabinet under soda dispenser and a few areas of the kitchen. Must take actions to minimize mice and clean up area.
    • Observed: There were a number of refrigerated, ready-to-eat, potentially hazardous foods in refrigerator that lacked any date marking: sushi, cooked chicken, some sauces, etc…
    • Observed: The final rinse at 3 vat sink did not have any added chlorine to the solution. Must be 50-100 ppm chlorine and have contact time.

    • Observed: Floor drain in facility near 1 door prep table is clogged and unable to properly drain grey water. Blockage is resulting in standing grey water. PIC stated employees and currently squeegeeing grey water to a functioning floor drain. Facility is required to have repairs made to floor drain in 24 hours or a closure until repairs can be made will be enforced.

    • Observed: Steam table has active leak. Patch repair was made with duct tape and is insufficient. Leak is resulting in standing water in the facility. Education Provided.
    • Observed: AC unit is producing large amount of condensation resulting large amount of water dripping from ceiling tiles. Dripping water amount is significant enough to cause standing water inside dry storage area and chicken prep area. No food contact surfaces are currently affected by dripping. Repairs need to be made to remove potential sources of contamination and stop the creation of standing water.
