EUGENE, Ore. – The Lane County Sheriff’s Office is investigating a fatal crash that occurred on the Northwest Expressway south of Park Avenue in Eugene on October 15. Deputies responded to the scene at approximately 7 a.m., alongside Eugene Police and Eugene Springfield Fire.

The crash involved a white 2013 Toyota Sienna minivan and a tan 2007 Chevy Trailblazer. Initial investigations revealed that the Chevy was traveling south when it crossed into the oncoming lane and collided with the northbound Toyota.

The driver of the Toyota, identified as Amy Lyn Samson, 55, of Eugene, was pronounced dead at the scene.

The driver of the Chevy, identified as Daniel Eugene Pina, 37, of Eugene, and a passenger in the Chevy were transported to a local hospital. Pina later died from his injuries. Investigators believe Pina may have suffered a medical event while driving, leading to the crash. Neither Pina nor his passenger were wearing seatbelts, while Samson was.

The identities of the passenger and others involved were initially withheld until next of kin could be notified.

The roadway was closed for several hours as deputies conducted their investigation, and tow companies worked to clear the scene. The Lane County Sheriff’s Office expressed its appreciation to the Eugene Police Department and Lane County Public Works for their assistance in managing the road closure during the investigation.
