Royal Farms, 2401 Mechanicsville Turnpike – The following violation was reported during a routine inspection: temperature of spicy chicken tenders in front service warmer unit is too low.
Kabab House, 3411 Old Parham Road – The following violations were reported during a routine inspection: temperatures of noodles and several containers of chicken in walk-in cooler are elevated; temperature of milk is elevated because it was left out on the counter; foods throughout the kitchen are not date marked; personal medicines are stored next to food on spice racks; observed one live roach crawling on floor under spice racks and a large amount of dead roaches under three-compartment sink and prep sink; two white cutting boards have deep scratches; a leak is coming from the ceiling of reach-in cooler near white fridge; soiled cardboard is lining bottom shelf of prep counter; there is grime buildup on wire racks throughout kitchen; observed heavy mold buildup on wire racks in reach-in cooler; observed grease/food buildup on exterior of units on cook line; there is a leak at three-compartment sink handle and customer hand sink near front counter; floors under grill, three-compartment sink and prep sink are dirty; ceiling light shield near naan oven is cracked.

Park Lane Tavern, 2251 Old Brick Road – The following violations were reported during a routine inspection: dishwash area hand sink doesn’t have hand soap; hand sinks in expo and dishwash areas don’t have paper towels; dried food debris found on can opener blade; temperature of top portion of gravy in steam well is too low; temperatures of milk and cheese in expo prep unit are elevated; brunch menu doesn’t contain a consumer advisory/disclosure statement for poached eggs that may be served raw and/or undercooked; observed several small flies in kitchen; in-use knife is stored in a dirty crevice between prep units; metal wires are bent on fryer baskets; small prep unit cover is broken; all prep and single cutting boards are heavily scratched and stained brown; observed buildup of grease and food debris on surfaces of suppression nozzles, pipes and cables behind cook line, floor behind cook line, blue dish racks, and wire storage racks; there is old label residue on containers; there is debris in floor drain at bar.
Westbury Deli, 8903 Three Chopt Road – The following violations were reported during a routine inspection: bowl found inside hand sink; cart is blocking sink; open package of sliced deli meat is not date marked; observed small fly infestation near hand sink/three-compartment sink; metal wires are broken off on fryer baskets; reach-in cooler storing bread is in disrepair; facility doesn’t have an irreversible registering temperature indicator for hot water mechanical warewashing operation; observed grease and food buildup on legs and sides of grill; food debris buildup found on floor behind prep units and under grill.
Mezeh Grill, 6927 Forest Avenue – The following violations were reported during a routine inspection: person in charge couldn’t provide food manager certificate; temperatures of spinach, shredded cheese and lamb kefta in sandwich unit are elevated because foods are stored in large, deep, covered containers; reach-in unit for drinks isn’t working; wire racks next to dish machine and in walk-in cooler are rusted.
Taco Bell, 8991 Staples Mill Road – The following violation was reported during a routine inspection: observed ice buildup in top of reach-in freezer because gasket is loose on bottom door.

Revel, 6223 Lakeside Avenue – The following violations were reported during a routine inspection: employee touched face and didn’t wash hands before handling food; pan of boiled eggs found inside hand sink; hand sink near dish machine doesn’t have paper towels; the date when the last mussell was sold/served isn’t recorded on tag; dried food debris found on can opener blade; temperatures of crab fritter, pork belly, smoked tomato aioli, calabreso and raw filling in walk-in and reach-in coolers are elevated; menu doesn’t contain a consumer advisory for foods that may be served raw and/or undercooked; sanitizer bottle at bar is unlabeled; an employee working in the food service area isn’t wearing proper hair restraints; prep and single cutting boards at bar are heavily scratched and stained; metal wires are bent on fryer baskets; observed dried food debris and grease buildup on surfaces of dry dish rack, ovens, walk-in cooler racks and under grill; there is trash and grease on the ground around the outside dumpster and grease receptacle; observed food debris and grime buildup on floor at bar; ceiling and vents are dusty; there is heavy buildup on hood filters, especially above grill.
Jus Crab Bar & Grill LLC, 2204 East Laburnum Avenue – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; menu has consumer advisory but animal-derived foods requiring disclosure are not marked with an asterisk; menu states catfish nuggets but swai nuggets are being presented and sold; beer cooler at bar has a torn gasket; wire racks throughout kitchen are rusty; fryer baskets are in disrepair; facility doesn’t have sanitizer test strips; walk-in cooler fan guards, fire suppression system tank and exterior of ventilation hood are dusty; heavy grease residue found on sides of hot line cooking equipment; top of dish machine and gaskets of beer cooler at bar are dirty; observed a leak underneath three-compartment sink; there are food stains on walls throughout kitchen; there is grease on hot line floor; observed mold/mildew on wall behind three-compartment sink and on floor below; observed several holes in tiled wall near three-compartment sink.
Roots Natural Kitchen—Richmond, 6602 West Broad Street – The following violations were reported during a routine inspection: metal pan used to hold clean utensils is dirty; temperature of chopped kale in walk-in cooler, unit at remote order station and reach-in cooler is elevated; temperature of bulgur in reach-in cooler is elevated because of improper cooling methods (stored in deep pan covered tightly with several layers of plastic wrap); air temperatures of both walk-in cooler and reach-in cooler at remote order station are elevated; dishplate in dish machine isn’t measuring the temperature correctly; all-gender restroom on right side doesn’t have a covered trash can; there is dust buildup on ceiling panels in dish area; observed grime buildup under hand sink in back room and on floors under serve line; mold buildup found on cooling unit under serve line.

Yen Ching, 9031-9035 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide food manager certification; hot water isn’t working at kitchen hand sink; temperatures of sprouts, noodles, sliced tomato, beef and chicken in two prep units closest to woks are elevated; this unit isn’t working properly; rice and fried chicken in bowls near woks are not at the proper temperature; burn gel and eye drops are stored above prep units; flour, sugar and salt in large bins on floor and in small bowls on cart are unlabeled; rice scoops are stored in ambient temperature standing water; long white cutting board at prep unit is heavily scratched; chipping paint found on dish storage racks and wire rack baskets under soup warmers; prep unit nearest reach-in cooler has a leak and one door is unable to fully close; thermometers in both walk-in units are not working; observed food buildup on cart at wok station; there is a leak at bar hand sink; one light is out at hood; floor tiles near fryers and back door are cracked; observed low grouting in dish area; floor in walk-in freezer is lifted and uneven; observed grease buildup on floors, wall and hood at wok station.

Domino’s Pizza, 8703 West Broad Street – The following violations were reported during a routine inspection: two chemical spray bottles are unlabeled; there is a leak at pipe under hand sink; observed low grouting throughout kitchen.
Olive Garden Italian Restaurant, 7113 West Broad Street – The following violations were reported during a routine inspection: bar hand sink doesn’t have paper towels; observed mold inside soda gun at bar; a pesticide found in chemical closet; quat sanitizer level at bar three-compartment sink is too high; records required as part of the HACCP plan are not being maintained in a consistent manner; there is inaccurate/nonexistent logging of temperatures; employee with fingernail polish or artificial nails is handling exposed food without gloves; observed staff wearing loose jewelery while prepping food; sanitizer buckets are stored directly on kitchen floor; utensils on hot line are stored in water; clean dishes are stacked while still wet; single-service and/or single-use articles are not handled, displayed or dispensed so that contamination is prevented; storage shelf is dirty and greasy; meat walk-in cooler door has a torn gasket; produce walk-in cooler door and wall are rusted; sanitizer test strips are not readily available at bar area three-compartment sink; grease buildup found on sides of hot line equipment and reach-in cooler door on hot line; point-of-sale monitors, keypads and cords and shelves above expo area are dusty and greasy; salad cooler interior and gaskets and wine cooler door gaskets are dirty; observed food debris on walk-in cooler wire racks; there is grease residue on outside of grease receptacle; outdoor dumpster door was left open; two lights are out in ventilation system in back line; there are food stains on ceiling tile above hot line and on floors in walk-in cooler and at bar area.
