Yankees starter Luis Gil was wildly ineffective through much of Tuesday’s outing against the Guardians, and the 12-game winner departed the game in the fourth inning with what the team announced was lower back tightness.

Gil issued his sixth walk of the game — and third to leadoff man Steven Kwan — before calling for and exiting alongside a team trainer with a 1-0 count to Jose Ramirez with nobody out in the fourth of an eventual 12-inning loss .

The Yankees were trailing, 3-2, following a solo home run by Brayan Rocchio against Gil earlier in the inning.

“I’ve never felt anything like that, so yeah, you’re a little worried because you come out of the game because you’re feeling something that’s not right,” Gil said through a translator. “But at the same time, you gotta take it day by day and see how I feel tomorrow.”

Asked if he believes he could be headed for the injured list, Gil added, “I saw the doctor here, and he told me to take it day-by-day, so we gotta wait and see.”

The Yankees announced during the game that Gil was seen at the Stadium by Dr. David Trofa, the team’s orthopedic doctor.

He will be reassessed Wednesday, but the team said no imaging was scheduled.

Afterward, Aaron Boone said Gil previously wasn’t dealing with any back issues.

“It was just a struggle command-wise, just not in the strike zone a lot,” Boone said. “It’s a lower back. We’ll see. It didn’t seem too serious initially, but we gotta see what we got there.”

The 26-year-old Gil has thrown 124 ²/₃ innings this year after missing most of the previous two seasons due to Tommy John surgery in May 2022. He was working Tuesday on seven days of rest since his previous start last Monday against the White Sox.

“Stuff-wise, I think he’s in a really good spot. He feels very good, looks strong to me,” Boone said before the game. “Will the rest benefit him? I don’t know. Sometimes it can even throw you off a little bit, but I think over the long haul, it hopefully will serve him well.”

Gil worked a scoreless first inning and was staked to a 2-0 lead on back-to-back homers by Juan Soto and Aaron Judge in the bottom half. But the Yankees starter walked two in each of the next two innings, and the Guardians tied the score on an infield out in the second and an RBI single by Daniel Schneemann in the third.
