    Richland County restaurant inspections: July 2-8 critical violations

    The following restaurant inspections with critical violations were conducted by Richland Public Health between July 2 and 8:

    JOANN #975 , 594 Lexington Springmill Road, Mansfield, July 2. Improper storage of poisonous or toxic materials (critical). Observed paint and tie dye kits stored above prepackaged food. Correct by July 9.

    Liberty Nursing Center of Mansfield , 535 Lexington Ave., Mansfield, July 3. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical repeat). Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected during inspection). Observed no signed employee illness agreement for employee. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected).

    Observed two buckets of chlorine sanitizing solution to have a concentration less than 50 ppm. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical repeat, corrected). Observed chorine dishwashing machine to have a concentration less than 50 ppm. Facility stated they will be discontinuing unit until it can work properly and will be utilizing the three-compartment sink. Repair the machine by reinspection July 10. TCS foods not being cold held at the proper temperature (critical, corrected). Observed milk on the cart that was returned from the dining room to have an internal temperature of 44-45 degrees.

    Currently the facility is utilizing an ice bath for the drinks while serving. The facility is to discontinue this practice until milk can be stored in complete temperature control the entire service time. Presence of live rodents (critical repeat, corrected). Observed rodent droppings in the corner of the kitchen near the mixer and in the back dry storage area behind the door. Facility was able to produce the last three months worth of pest control reports. Facility is to continue sending reports until October, contact pest control operator and thoroughly sanitize the area. Correct by reinspection July 10. Insufficient air gap between the flood rim and the water supply inlet (critical, corrected). Observed insufficient air gap under the prep sink.

    Wendy's Old Fashioned Hamburgers , 653 N Lexington Springmill Road, Mansfield, July 3. TCS foods not being cold held at the proper temperature (critical repeat, corrected during inspection). Observed TCS (time/temperature controlled for safety) foods held above 41 degrees for an undetermined amount of time: sliced tomatoes, pre-made Caesar salads and cut lettuce. PIC voluntarily discarded sliced tomatoes and cut lettuce. PIC voluntarily placed all varieties of pre-made salads under temporary time in lieu of temperature hold to be discarded by 1:30 p.m. Improper use of time as a public health control - four hours (critical, corrected).

    Observed blue cheese on make line not time stamped to indicate discard time as per written time in lieu of temperature policy. PIC verified time blue was pulled from the cooler. Presence of live insects, rodents and other pests (critical). At reinspection, continued to observe small flying insects inside cabinet located at fountain beverage station in dining area, as well as near mop sink and warewashing area. Discussed thoroughly cleaning and removing all visible residues and reaching out to pest control to aid in removal of observed pest. Correct by July 10.

    Line Star Enterprise LLC dba The Electric Company , 1400 W. Fourth St., Mansfield, July 5. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). At reinspection observed no change. Correct by next reinspection July 12. Presence of live insects (critical repeat). Observed small crawling insects to be in the warewashing area. At reinspection, observed no change. Facility is to contact a pest control operator, remove all evidence of insects, completely disinfect the area and have pest control reports available for review during July 12 reinspection.

    Springmill Drive-In Theater , 1040 Springmill St., Mansfield, July 5. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). At reinspection, observed no change. Correct by next reinspection July 12. Presence of live rodents (critical, corrected during inspection). Observed two live rodents and droppings within the facility. During reinspection, observed droppings in cabinets. PIC did have cleaned and disinfected at time of inspection. Presence of live insects (critical repeat). Observed large flying insects throughout the facility. At reinspection, observed less flying insects. Facility is to rid all evidence of insects, thoroughly clean the area, contact a licensed pest control operator and have PCO reports/plan available for review by July 12 reinspection.

    Nelly's Hideaway LLC , 3717 Park Avenue West, Ontario, July 5. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Correct by reinspection July 12. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC stated that fryer baskets that are not held at temperature are cleaned once every 24 hours. If these baskets are to be held without temperature control, they must follow a four-hour cleaning frequency. Equipment food-contact surfaces or utensils are unclean (critical, corrected).

    Observed tongs and fryer baskets with visible residue. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected. Observed various items throughout the facility that were not date marked. PIC had all items either date marked or voluntarily discarded. Presence of live insects (critical). Observed small flying insects on the men's urinal. Facility is to contact a pest control operator, retain PCO report/summary for review, thoroughly clean the areas and completely eliminate the insects by July 12 reinspection.

    Wingate By Wyndham , 2069 Walker Lake Road, Ontario, July 8. Handwashing sink not accessible (critical, corrected during inspection). Observed bag of egg omelettes and coffee filters stored on counter directly in front of hand washing sink, blocking access to hand sink.

    Taco Bell #24647 , 2085 Walker Lake Road, Mansfield, July 8. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed bags of shredded cheddar to have an internal temperature of 43 degrees.

    Subway #12061 , 950 Ashland Road, Mansfield, July 8. Handwashing sink not accessible (critical, corrected during inspection). Observed food preparation items located around hand sink. Presence of live insects, rodents and other pests (critical). Observed flies in food production area. Facility shall obtain some form of fly control. Correct by reinspection July 15.

    Papa John's Pizza #802 , 201 S. Main St., Mansfield, July 8. Presence of live insects, rodents and other pests (critical). Observed flies in facility. Correct by July 15.

    Gionino's Pizzeria , 176 B E. Main St. B, Lexington, July 8. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected during inspection). Observed containers of coleslaw on a tray that were not date marked. Ready-to-eat, TCS food not properly discarded when required (critical, corrected). Observed a container of cooked chicken in bags with a use by date of July 7. PIC voluntarily discarded bags of chicken. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed a spray bottle of degreaser in the mop closet that was not labeled.
