HENDERSON, Ky. (WEHT) – The Indiana Department of Homeland Security has provided an updated list of safety violations that were noted during an inspection on Tuesday.

Investigation begins after a girl falls off a ride in Dubois County

On June 11, two of the rider carriages on the Rock and Roll ride were deemed unusable and were not utilized in Dubois County. However, officials say that during the inspection on July 16, the repairs to the two original carriages had not been completed and three additional violations were noted:

    • General maintenance violations
      • Missing drive tires, missing belt fasteners and body damage to rider carriages, including holes that can be pinch points.
    • Assembly/welding
      • Diamond plate flooring severely warped in multiple places, broken rivets and screws.
    • Electrical safety
      • Multiple light sockets with no working bulbs/blanks.

For these reasons, the ride was deemed unusable until it passes an IDHS compliance inspection.
