Court officials said a DEA inspection in 2018 revealed inconsistencies and the co-mingling of controlled substances among different registrants. The DEA completed similar inspections in 2021 and 2023 that showed a continuation of recordkeeping violations, although the facility said it would create an electronic recordkeeping system in 2018.

After the settlement agreement for the Controlled Substance Act violations, Conifer Park said it would implement an electronic recordkeeping system by Nov. 1, 2024. DEA Albany District Office’s Diversion Group investigated the case.
