when I was welding in the oilfield this shit happened all the time. almost every confined space I entered had some sort of safety aspect overlooked. don't even get me started on the guys at the jobsite calling you a "p***y" for not getting inside the vessel anyways. Surprised more people havent died. This type of stuff happens all day everyday. the people just don't always die.
