Moffatt named Dementia Outreach Specialist
Thrive Alliance has appointed Jennifer Moffatt to the position of Dementia Outreach Specialist. In her new role, Moffatt will be responsible for implementing Indiana’s Dementia Care Specialist Program (DCSP) for Thrive Alliance’s PSA (Planning and Service Area). With the goal of improving Indiana dementia-related population health outcomes, the DCSP supports Indiana’s transformation into a dementia capable state equipped with dementia capable care and services for all Hoosiers. Special attention will focus on strategies to decrease health disparities concerning dementia in Indiana.
