    Health survey takers wanted

    SPOONER — Spooner Health, Washburn County Public Health and Indianhead Medical Center are teaming up to conduct a Community Health Needs Assessment. The assessment is a short survey that collects information from Washburn County residents and surrounding communities to better understand and address the health needs of its communities.

    The survey is available at spoonerhealth.com/chna-survey/. Paper copies are also available at the following locations:

    Indianhead Medical Center & Shell Lake Clinic

    113 Fourth Ave. W, Shell Lake

    Minong Area Library

    212 W. Fifth Ave., Minong

    Shell Lake Public Library

    501 First St., Shell Lake

    Siren Clinic

    24670 Hwy. 35/70, Suite 100, Siren

    Spooner Health

    Spooner Memorial Library

    421 High St., Spooner

    Washburn County Health Department

    304 Second St., Shell Lake

    The survey period is open now through Oct. 15. You must be 18 years of age or older to take the survey, and you can only complete one. If you have questions about the survey, contact Michelle Martin, Public Relations & Marketing Director, at Spooner Health at 715.939.1736.

    Once the survey is closed, the results will be used to crate a three-year implementation plan with partnering agencies throughout the county.
