    These Greene County establishments had no health violations this week

    Almost a third of the establishments inspected by the Springfield-Greene County Health Department had no violations this week, according to reports.

    Of the 46 routine inspections, 14 establishments had no violations cited at all.

    Food inspections take place one to three times a year, depending on the type of food served, the population served, difficulty of food preparation and past history. Restaurants preparing food from raw ingredients are inspected more often "than convenience stores that serve only non-potentially hazardous foods, such as popcorn and soda," according to the Springfield-Greene County Health Department.

    Each report identifies what was happening at the establishment at that time. Consumers who want to know whether a violation is a one-time thing or a pattern are encouraged to refer to previous reports.

    Issues found during inspection fall into either priority or non-priority violations. Priority violations impact the safety of the food, such as cross contamination between raw and ready-to-eat food, improper food temperature and poor personal hygiene and employee health. Multiple priority violations can lead to an establishment being shut down. Non-priority violations alone do not directly affect food safety, such as dirty floors, sticky tabletops or outside trash cans not being covered.

    4D's Eats LLC, 91 S. Main St., Fair Grove. July 15 routine inspection – Result: active. No violations observed.

    Alli's Family Restaurant, 4525 W. Chestnut Expressway, Springfield. July 15 routine inspection – Result: permitted with re-inspection. One priority violation: Observed several gnats around dish line and trash can. Four non-priority violations: Observed shelf above dish line, some areas of kitchen floor and dry storage area were in poor repait; observed floors in dry storage, dish room and kitchen, shelves and walls in walk-in refrigerator and walls in dish room were not clean; observed exterior of toasters, handles to plastic silverware drawers and some carts were not clean; observed range hood had heavy buildup.

    AM PM Food Mart - Phillips 66, 2720 W. Kearney Ave., Springfield. July 15 routine inspection – Result: active. Two priority violations: Observed several small bottles of yogurt drink with expiration dates of April 5, 2024, and May 24, 2024, which was corrected on site by discarding the drinks; observed numerous containers of hair color, shampoo, antiseptic and liquid hand washing stored on shelves above bags of flour, which was corrected by relocating prior to exit. Five non-priority violations: Observed wall and ceiling by mop sink is in poor repair; observed several bozes of produce stored on floor in walk-in refrigerator and some boxes of food stored on floor of walk-in freezer; observed some boxes of single-service items sitting on floor near restroom; observed staff unable to fund any quaternary ammonia test strips for checking sanitizer; observed fan covers in walk-in beer cooler were coated in debris.

One priority violation: Observed multiple items in top of make table not at or below 41 degrees, which was corrected by voluntarily discarding product. Three non-priority violations: Observed pipe under three-vat sink has leak in center part; observed test strips for dish machine are expired and there are no test strips for ammonia sanitizer; observed floor behind the second fryer has a hole allowing for accumulation of grease.

One priority violation: Observed in-house made horseradish sauce had not date labeling, which was corrected on site and education given. One non-priority violation: Observed hot hold cabinet door seal near prep line was damaged, education given.

    Buffalo Wild Wings - Patton Springfield Opco LLC, 900 E. Battlefield Road, Springfield. July 15 re-inspection – Result: active. Re-inspection conducted and three-vat sink sanitizer dispenser is repaired and operating properly. No violations cited.

    Catrina's Cantina LLC, 2925 E. Battlefield Road, Springfield. July 16 routine inspection – Result: active. No priority violations. One non-priority violation: Observed no test strips at the bar and expired test strips in the kitchen area, education given.

    Daylight Donuts, 516 N. West Bypass, Springfield. July 16 routine inspection – Result: active. There were a few gnats around mop sink, establishment will continue to take actions to minimize them. No priority violations. Three non-priority violations: Observed mop sink and outside of proofer were not clean; observed spring on bathroom door was not attached; observed some areas of the floor in food preparation area were not clean.

    El Taco, 316 W. Kearney St., Springfield. July 16 routine inspection – Result: active. No priority violations. Two non-priority violations: Observed light fixture in area with three-vat sink and prep table has a leak, leak drips to floor directly next to prep table and A/C vent above register area has condensation on it, so duct work in upstairs has a hole allowing for cold air to escape; observed buildup of food debris and spatter on kitchen walls and ceilings and wall around window air conditioning unit in kitchen has buildup around it.

    Homewood Suites, 4340 S. National Ave., Springfield. July 12 re-inspection – Result: active. Violations corrected prior to re-inspection. No violations noted.

    Hy-Vee - Italian & Chinese, 1720 W. Battlefield Road, Springfield. July 15 routine inspection – Result: active. One priority violation: Observed Chinese food prep table had cooked chicken, cooked pork and raw beef with temperatures between 49 degrees Fahrenheit and 55 degrees, which were voluntarily discarded and the sushi prep table had spicy tuna, salmon, imitation crab and raw shrimp at temperatures between 44 degrees and 47 degrees, which were discarded by the manager. No non-priority violations.

    Hy-Vee Deli Kitchen/Club Room, 1720 W. Battlefield Road, Springfield. July 15 routine inspection – Result: permitted with re-inspection. A re-inspection is scheduled for July 18. One priority violation: Observed sour cream, salsa, sressings, mixed fruit and chocolate milk in condiment cold holder at 45 degrees to 47 degrees.     Mundos Latin Restaurant LLC, 2550 S. Campbell Ave., Springfield. July 10 routine inspection – Result: active. Three priority violations: Observed multiple dented cans in storage location on top of cooler, education provided and corrected on site by setting cans aside to return to vendor; observed pico de gallo at 65 degrees in prep cooler with ambient air at 68 degrees, all potentially hazardous food voluntarily discarded and re-inspection required; observed knives hanging on knife rack had visible food debris on knife blade and handle, education provided and corrected on site. No non-priority violations.

    Nicola's Ristorante, 3631 E. Sunshine St., Springfield. July 11 routine inspection – Result: permitted with re-inspection. Two priority violations: Observed spray bottle unlabeled with sanitizer, education given and corrected on site; observed all cold-holding units were unable to hold 41 degrees or less, possibly due to air conditioning unit being out of service, multiple items were voluntarily discarded and re-inspection date set. Five non-priority violations: Observed ragu and marinara sauce cooling in large plastic containers in walk-in cooler, corrected on site by transferring product to shallow metal pans; observed beef cooling in large metal pans with foil partially pulled back from one side in walk-in cooler; observed both make tables, reach-in cooler and walk-in cooler did not have thermometers located inside to reach ambient temperatures; observed seal is damaged on left door of make table 1; observed several pieces of tile missing on wall between make table 1 and oven.

One priority violation: Observed hard-boiled eggs at 47 degrees and cut strawberries at 46 degrees in lower portion of prep table No. 3, both items were discarded by manager and any food in prep cooler moved to walk-in cooler. Re-inspection scheduled for July 15. One non-priority violation: Observed mouse carcass in cabinet under soda dispenser, corrected on site by notifying manager who will discard mouse sanitarily after inspection.

    Price Cutter No. 85 - Retail and Produce, 163 W. Old Mill Road, Fair Grove. July 15 routine inspection – Result: active. No priority violations. Two non-priority violations: Observed various produce items cut and wrapped on site do not have establishment information on label; observed produce department does not have test strips for sanitizer.

    Price Cutter Plus No. 50 Bakery/Retail, 3260 E. Battlefield Road, Springfield. July 16 routine inspection – Result: active. One priority violation: Observed unlabeled spray bottle in production bakery containing surface celaner, which was corrected on site and education given. Two non-priority violations: Observed ice buildup in production bakery freezer and a repeat issue of standing water on the floor inside the small walk-in cooler in production bakery, education given on both; observed gap on bottom of exterior door in production bakery, education given.

    Price Cutter Plus No. 50 Fonnies Tap, 3260 E. Battlefield Road, Springfield. July 16 routine inspection – Result: active. One priority violation: Observed quaternary sanitizer solution in sanitizer bucket testing above 400 ppm, which is a repeat issue that was corrected on site and education given. Discussed labeling working bottles of cleaners and food product and keeping hand sink unblocked to be accessible for hand-washing. Four priority violations: Observed dish machine not dispensing sanitizer and person in charge was not able to correct on site so three-vat sink will be used to wash, rinse and sanitize items until dish machine can be services; observed sanitizer exceeding 400 ppm in three-vat sink, which was corrected on site; observed glass single-door cold-holding unit had ambient temperature of 51 degrees, so multiple potentially hazardous food items were voluntarily discarded and education was given, re-inspection date set; observed multiple ready-to-eat potentially hazardous food items not dated, like cut purple cabbage, spicy crab meat, cut lettuce, cut tomato, green curry sauce, red curry sauce and shrimp tempura, were not dated, which was corrected on site. One priority violation: Observed employee cutting lemon and lime wedges with bare hands, which was corrected on site by voluntarily discarding food items and having employee don gloves. Two non-priority violations: Observed several sleeves of single-use cips sitting on floor next to the ice machine, several sleeves of single-use shot cups on floor behind main bar area; observed seal on reach-in cooler's right door is damaged.

    W.F. Cody's, 3138 E. Sunshine St., Springfield. July 15 routine inspection – Result: active. One priority violation: Observed coleslaw at 53 degrees, potato skins at 51 degrees, cabbage at 52 degrees in walk-in cooler, which were discarded and a re-inspection will be conducted. No non-priority violations.

    Wendy's No. 1061, 4939 W. Chestnut Expressway, Springfield. July 12 routine inspection – Result: permitted with re-inspection. Re-inspection will be made on or after July 17. Facility had a few ceiling tiles that needed to be replaced due to water damage, staff said roof had just been repaired. Three priority violations: Observed hose attached at mop sink without back-flow preventer, which is a repeat violation; observed numerous salads prepared in establishments are kept 48 hours but not labeled with use-by date; observed quaternary ammonia jug was empty and none was showing up in test strip, which was corrected by staff getting another container and manually setting up sink. Four non-priority violations: Observed floor by walk-in refrigerator, drive-up window, back area and some tables in dining room were not clean; observed the area around self-service drink dispenser heads and ones by drive thru were not clean nor was the lower portion of soda dispensers; observed several boxes of food on floor of walk-in freezer; observed several boxes of single-service food containers and gloves being stored directly on the floor.

    White Oak Station No. 61, 650 W. Kearney St., Springfield. July 12 re-inspection – Result: active. Walk-in cooler is still not producing temperatures of 41 degrees or below, owner will notify health department once repairs have been made to unit. No priority violations. One non-priority violation: Observed walk-in cooler not producing temperatures of 41 degrees or below and potentially hazardous foods not being stored in unit.
