    Sushi, Mexican eateries score 45 red points. Here are Thurston food safety scores for Oct. 9

    Comments are taken directly from the latest Thurston County Public Health and Social Services (PHSS) inspection reports, which are available for each food establishment, at the Thurston County Environmental Health Office and their website .     Reading inspection scores:

    • Attention should be given to the type of violation (red versus blue) and whether the establishment has a history of red violations or repeat violations.
    • Red violations are those most likely to cause food-borne illness and must be corrected immediately if feasible or according to a compliance schedule established by the health officer. Example: not keeping food at the right temperature.
    • Blue violations relate to the overall cleanliness and condition of operation and must be corrected according to a compliance schedule established by the health officer. Example: a worn floor that needs replacing.
    • If red points exceed 45, the county will require a reinspection within 10 business days. A reinspection also is required if total red and blue points exceed 65.
    • If red points exceed 100 and there’s a lack of managerial control, the county will shut down the establishment. The establishment may reopen after the person-in-charge develops a written course of action to correct all violations and demonstrates corrective action during a pre-opening inspection. Closures also may occur if there is an imminent health hazard, regardless of the total inspection points. The length of a closure may vary depending on the violation and timeframe for corrective actions.
    • Some common abbreviations: PHF/TCS is potentially hazardous food/time control for safety; RTE is ready to eat; PIC is person in charge; FWC is food worker card; CDI is corrected during inspection.

    Comments:

    • The person-in-charge lacked basic knowledge to perform duties and maintain active managerial control of risk factors.
    • A hand-wash sink was not equipped with soap.
    • Workers did not use time stamp tracking for sushi rice.
    • The make line had an improper cold-holding temperatures. Various TCS food items were above 41 degrees.
    • The in-use chlorine solution was too weak as prepared.

    Mayan Family Mexican Restaurant

    2120 Marvin Road NE, Lacey

    Oct. 1: 45 red points; 0 blue points.

    Comments:

    • Raw chicken was stored over raw beef in the walk-in cold hold. Raw animal products must be stored in order of cooking temperature and below ready-to-eat foods.
    • Cooked peppers and onions on a grill measured 119 degrees. Hot held TCS foods must at or above 135 degrees.
    • Salsa and cheese on ice measured warmer than 41 degrees. Cold held TCS foods must at or below 41 degrees.
    • No probe thermometer was available during the inspection.

    Little Caesars Pizza

    Sept. 30: 35 red points; 0 blue points.

    Comments:

    • A certified food protection manager certificate was expired.
    • Two employees were unable to present food worker cards.
    • Pizza and other TCS food items for donation were improperly cooled.

    Oct. 2: 10 red points; 13 blue points.

    Comments:

    • TCS foods measured at 44 degrees. Cold held TCS foods must be at or below 41 degrees.
    • A cold hold unit lacked a thermometer.
    • One non-commercial cold hold unit was used.
    • Flies were present throughout the facility. The inspector instructed the establishment to seal the back door near a two-door cold hold and create a pest management plan.
    • Food debris was observed throughout the facility.

    Taco Bell

    Sept. 27: 10 red points; 5 blue points.

    Comments:

    • A certified food worker protection manager was absent at the time of inspection.
    • Food worker cards were missing or unavailable.
    • A plumbing back-flow device was not tested annually as required. A hand sink provided water that was too hot.

    Comments:

    • Vanilla milk shake in a machine measured at 43-55 degrees. All cold held TCS food must be kept at or below 41 degrees.
    • A sanitizer solution was too weak as prepared.

    Comments:

    • An employee was observed rinsing and washing grapes in a three-compartment ware-washing sink. Employees must use a designated food sink.
