    The Lucas County Coroner has ruled in the following deaths:

    Robert Irwin, 95, of Liberty Center, Ohio, Feb. 18, at ProMedica Toledo Hospital. Accidental, complications of reinjuring a previously fractured right hip; deceased fell at home.

    Charles Johnson, 53, of Toledo, Aug. 8, at home. Accidental, fentanyl toxicity.

    Joslyn Jones, 19, of Oregon, Feb. 7, on I-280 southbound. Accidental, multiple blunt-force injuries sustained from vehicle accident.

    Christopher Kaminski, 70, of Toledo, Oct. 11, at home. Undetermined.

    Melvin Keyes, 84, of Toledo, Jan. 31, at Ebeid Hospice in Sylvania. Accidental, rhabdomyolysis with multiorgan failure due to left thigh injury; deceased fell at extended care facility.

    Douglas Kirsch, 47, of Toledo, Oct. 10, at home. Accidental, combined drug toxicity (alparazolam, fentanyl, and hydrocodone).

    Lori Klein, 61, of Toledo, Sept. 15, at Mercy Health St. Vincent Medical Center. Accidental, multiple blunt-force injuries; deceased was struck by vehicle.

    Annie Kubiak, 93, of Sylvania, Feb. 19, at University of Toledo Medical Center. Accidental, blunt trauma with right femur and right radius fractures; deceased fell.

    John Kuch, 52, of Toledo, Feb. 7, at Hospice of Northwest Ohio on South Detroit. Accidental, blunt head trauma and complications; deceased fell from bed.

    Andrew Kulczak, 33, of Toledo, Sept. 19, at home. Accidental, combined drug toxicity (acetyl fentanyl and fentanyl).

    Dean Lamont, 56, of Toledo, Nov. 27, in 2300 block of West Alexis. Accidental, hypothermia.

    Ricky Leist, 71, of Wauseon, Oct. 29, at ProMedica Toledo Hospital. Accidental, craniocerebral injuries due to deceased falling at home.

    Carole Loth, 86, of Toledo, Jan. 5, at University of Toledo Medical Center. Accidental, positional asphyxia, driver was entrapped in single vehicle accident.

    Shaun Mancini, 45, of Waterville, Oct. 26, at home. Accidental, combined drug toxicity (cocaethylene, cocaine, and fentanyl).

    Scott Marckel, 30, of Toledo, Oct. 21, at home. Accidental, combined drug toxicity (fentanyl and methamphetamine).

    Jeffrey Mashburn, 69, of Temperance, Feb. 15, at Mercy Health St. Vincent Medical Center. Accidental, blunt-force injuries associated with multiple falls.

    Jaquay Massey, 29, of Toledo, July 16, at home. Accidental, combined drug toxicity (fentanyl, cocaine, and cocaethylene).

    Jeannine Meeker, 92, of Waterville, Feb. 8, at home. Accidental, blunt-force head injuries; deceased fell at extended care facility.

    Ryan Miller, 45, of Perrysburg, Oct. 21, at University of Toledo Medical Center. Accidental, fentanyl toxicity.

    Charles Parkes, 71, of Toledo, Feb. 19, in the 1500 block of West River, Maumee. Suicide, incised wound to the left wrist.

    Christen Patton, 39, of Toledo, Oct. 12, at home. Accidental, combined toxicity (cocaine and fentanyl).

    Jessee Petit, 30, of Toledo, Feb. 7, at interstate 280 southbound. Accidental, multiple blunt-force injuries sustained from vehicle accident.

    Jarvis Phillips, 29, of Toledo, Oct. 8, in 700 block of Oak. Accidental, fentanyl toxicity and cocaine.

    Henry Randolph, 55, of Toledo, July 11, in 800 block of North Michigan. Accidental, fentanyl usage (advanced decomposition).

    David Rejiester, 68, of Toledo, Dec. 11, in 2500 block of Seaman. Accidental, arteriosclerotic cardiovascular disease.

    Jayde Roaden, 27, of Toledo, Sept. 12, at home. Accidental, fentanyl toxicity with acetyl fentanyl.

    Ashley Sanders, 30, of Toledo, Oct. 28, at home. Accidental, polypharmacy intoxication.

    Anthony Sanders, 83, of Maumee, Dec. 17, at Addison Heights Health and Rehabilitation Center in Maumee. Accidental, left humerus fracture exacerbating cardiac comorbidities; deceased fell in extended care facility.

    Karen Seguin, 77, of Maumee, Sept. 19, at Mercy Health St. Vincent Medical Center. Accidental, multiple blunt-force injuries sustained in vehicle accident.

    Teddy Shrader, 57, of Toledo, Aug. 14, at home. Accidental, polypharmacy intoxication including fentanyl.

    Alora Silva, 5 months, of Toledo, Oct. 6, at ProMedica Toledo Hospital. Accidental, asphyxia.

    Dianna Smith, 71, of Perrysburg, Feb. 1, at Hospice of Northwest Ohio on South Detroit. Accidental, blunt head trauma with subdural hematoma; deceased fell.

    Mark Tertel, 64, of Toledo, Jan. 14, at home. Accidental, smoke inhalation; deceased was in house fire.

    Mary Wells, 104, of Holland, Feb. 12, at Ebeid Hospice in Sylvania. Accidental, right femur fracture and complications; deceased fell in living facility.

    Ta’Lasiaa William, 12 months, of Toledo, Oct. 1, at Mercy Health St. Vincent Medical Center. Undetermined, unexplained sudden death.

    Earl Williams, 54, of Toledo, Nov. 20, at home. Accidental, combined drug toxicity (cocaine and fentanyl).

    Dennis Womack, 65, of Toledo, Dec. 18, at home. Accidental, arteriosclerotic cardiovascular disease; significant conditions cocaine intoxication.

    Rodney Wooden, 67, of Tiffin, Dec. 5, at Mercy Health St. Vincent Medical Center. Accidental, cocaine toxicity and complications.

    Loyalty Young, 3 months, of Toledo, Oct. 16, at ProMedica Toledo Hospital. Undetermined, unexplained sudden death.
