    Violations:

    7-Eleven , 3749 Upton, inspected June 11. The only employee on site at inspection did not know temperatures for safe hot or cold holding of temperature-controlled food or the significance of maintaining safe temperatures to prevent the spread of foodborne illness. A taquito on the roller grill was holding at an unsafe temperature. No person-in-charge qualified in food safety was present during inspection. The lone employee was not certified at any level of food safety. Repeat violation. Employees were not verifiably informed of duty to report illnesses relevant to food safety. In-use utensils including the pizza cutter, pizza server, and tongs were dirty with large food debris buildup. The metal peel used to handle foods into and out of the Turbo Chef were stored in an unsanitary location between uses and the peel’s surface was dirty. The hot dog roller’s food-contact surfaces were encrusted with buildup. The interior bottom of the hot dog bun storage drawer was dirty with food debris buildup. Two large containers of consumer-grade pesticide were stored in the back room’s dry storage area. Motor oil was stored improperly above food in the back stock/​dry storage area. The back stock room was overstocked and messy, making proper pest control difficult. Several roaches were on glue traps and dirt/​sand were present around baseboards, indicating a roach infestation. Gnats also were present, especially by the chili/​cheese machine and the self-service condiment area.

    Vito’s Pizza , 315 S. Detroit, inspected June 12. An employee failed to wash hands between return from a delivery run and resuming food handling. An employee did not wash hands upon returning work from smoking outside. Sanitizer buckets sat improperly on food-contact surfaces. The prep sink’s drain plumbing did not have a proper air gap to prevent sewage backflow.

    The Black Pearl , 4630 Heatherdowns, inspected June 12. Knives were stored improperly in the hand-washing sink by the ice machine. Meatballs in the two-door reach-in cooler by the prep line had expired. The server station ice bin by the kitchen coffee and expo station had no air gap in its drain plumbing to prevent sewage backflow.

    S&G , 1702 W. Laskey, inspected June 12. A mop head was stored improperly in the hand-washing sink.

    Hollywood Casino Toledo - Take 2 , 777 Hollywood, inspected June 12. A portable bar blocked access to the hand-washing sink. Quaternary ammonium sanitizer in the wipe bucket and in the three-bay sink tested at 0 ppm. Pizza and hot dogs in hot holding were not hot enough.

    Focaccia’s Delicatessen , 333 N. Summit, inspected June 12. The meat slicer was dirty with debris buildup. Chicken in hot holding was not hot enough. Desserts in the display cooler lacked required date marks.

    Woodlands at Sunset House , 4030 Indian, inspected June 13. Yogurt in the True single-door cooler was too warm. Food storage containers’ lids were cracked.

    Volunteers of America - AVI Food Systems , 1201 Champlain, inspected June 13. The vending machine for time/​temperature controlled food did not shut off automatically as designed to do.

    Tougen Japanese Restaurant , 7414 W. Central, inspected May 22. Raw fish was stored improperly above ready-to-eat foods in the two-door, under-counter cooler. Raw chicken was stored improperly next to raw fish in the walk-in cooler. Pork in the top of the prep cooler was too warm and stored improperly. Foods requiring date marks did not have them. Sushi rice holding on a time basis had no time marking.

    S&G , 2515 W. Laskey, inspected June 13. No hot water was available at the hand-washing, mop, or three-bay sinks.

    Sunset House - Main Kitchen , 4020 Indian, inspected June 13. Food containers and their lids were cracked.

    Ridis , 3841 Airport, inspected June 13. Chicken, chicken tenders, mashed potatoes, and macaroni in hot holding were not hot enough. Raw chicken in the walk-in cooler was too warm. Gnats were present.

    Norfolk Southern Railroad - AVI Food Systems , 343 Emerald, inspected June 13. Expired foods were not discarded.

    Neuro Science Lab , 2130 W. Central, inspected June 13. Pre-made sandwiches, open lunchmeat packages, cooked/​cooled chicken strips, and fresh mozzarella cheese lacked required date marks.

    Fritzie Freeze , 5137 N. Summit, inspected June 13. Food employees failed to wash their hands when changing tasks or before resuming food-handling after touching money. The supervisor failed to oversee effective hand-washing. Boxes blocked access to both hand-washing sinks. Flies were present throughout.

    Circle K , 1235 N. Holland Sylvania, inspected June13. The F’real machine had buildup inside beneath its center seal.

    Burger King , 910 Western, inspected June 13. The coffee spout was dirty.

    Babies N Beyond Childcare , 3125 Douglas, inspected June 13. Employees were not verifiably informed of duty to report illnesses relevant to food safety. Paint cans were stored improperly above open boxes of single-service items in the back storage room.

    Arby’s , 1425 S. Byrne, inspected June 12. Written procedures for preparing roast beef were not maintained. Employees did not know the safe food temperatures for hot and cold holding of food. No person-in-charge certified in food safety was present during inspection. An employee improperly touched manually operated faucet handles while washing their hands. A large amount of ice was dumped improperly in the hand-washing sink. Corned beef in cold-holding was too warm. Food containers had date stickers showing a day but not a date, making the meaning unclear. The ice machine’s drain plumbing had an insufficient air gap to prevent sewage backflow.

    Subway , 3324 Secor, inspected June 14. The ice machine’s interior was dirty with black buildup. Meatballs in the steam table were holding at an unsafe temperature.

    Poco Loco , 1809 Adams, inspected June 12. A water heater was not working properly, causing the low-temperature dish machine to not get hot enough during the wash cycle.

    Nada’s Not Your Average Kitchen , 5051 Yermo, inspected June 14. Raw salmon, tomatoes, and lettuce aboard the food truck were too warm.

    Hilton Garden Inn & Homewood Suites - Breakfast Kitchen Third Floor , 101 N. Summit, inspected June 17. A hand-washing sink was improperly filled with ice. The high temperature dish machine did not get hot enough to sanitize during final rinse.

    Bowlero Lanes , 4398 Monroe, inspected June 17. The ice machine’s slide and bin were dirty with a mold-like substance. Ham, cooked sausage, bacon, pizza sauce, and salsa in the prep-top coolers on the main cook line were held past their marked expiration dates.

    K & J House of Meats , 3821 W. Alexis, inspected June 18. Raw sausage was stored improperly above fully cooked sausage. Sanitizer in the wipe buckets was too weak to measure. Salads and lunch meats lacked required date marks. Deli meat was held past its marked discard date.
