    Wausau area restaurant inspections for June 2024

    Wausau Pilot & Review

    Monthly, Wausau Pilot publishes the results of health inspections at local bars and restaurants. When interpreting these results, it is important to note that there is a wide range of severity in health regulations. Many, if not most, violations are minor, ranging from cracked floor tiles to boxes improperly stored in a kitchen area. These types of violations are noted, but they clearly do not pose an immediate health risk to consumers. Others are core violations that are more consequential. Repeated violations are also noted. Though all violations are listed on the Marathon County Health Department’s reports, readers are urged to consider violations on a case-by-case basis.

    Inspections from June 1-30, 2024:

    19th Hole Tavern

    • Address: 1821 Grand Av, Schofield, WI 54476
    • Inspection Date: 6/13/2024. Two violations:
    • A dog was observed in the bar.
    • The ice scoop was improperly stored in the ice bin.

    AmericInn of Weston

    • Address: 4115 Barbican Av, Weston, WI 54476
    • Inspection Date: 6/7/2024. Three violations:
    • The small refrigerator in the kitchen is not properly cooling. Large boxes stored in the refrigerator may be obstructing the air flow.
    • Small kitchen refrigerator is not equipped with a temperature measuring device.
    • A strong pungent odor is emanating from the cupboard under the sink. The bottom of the cabinet has deteriorated and rotted away behind the grease trap. The grease trap was cleaned two months ago.

    Belvedere Supper Club

    • Address: 203630 State Highway 97, Marshfield, WI 54449
    • Inspection Date: 6/21/2024. No violations.

    Boriken Mart

    • Address: 5205 Lee Av, Schofield, WI 54476
    • Inspection Date: 6/8/2024. No violations.

    Christine’s

    • Address: 936 S 3rd Av, Wausau, WI 54401
    • Inspection Date: 6/12/2024. Four violations:
    • A number of non-shelf stable foods were found to be lacking date-marking, including laab sauce (3), garlic oil sauce, tamarind sauce, pho pepper sauce, wing pepper sauce, white rice in water, rice noodles, fresh chicken feet, cut onions, sliced jalapenos, and peeled garlic cloves.
    • Beef laab is offered undercooked but is not identified as such on the consumer menu.
    • A number of food-contact surfaces were found to be insanitary, including blades of kitchen knives on magnetic holder contacting wall with excess food residue, metal and plastic pans with food debris, debris on slicer, and cracks at bottom of wooden grinding mortar (discarded).
    • Numerous non-food contact surfaces were observed to be insanitary, including moderate mildew build-up on interior of ice machine, residue on wares rack, debris and masking tape on exterior of metal and plastic food containers, and debris on exterior surfaces of large metal mixing bowls.

    Culver’s Frozen Custard (Three locations)

    10202 Park Plaza, Rothschild, WI 54474 (Inspection Date: 6/19/2024). Six violations:

    • Food handling employee observed not washing hands prior to donning gloves.
    • Date-marking found to be absent on a number of cold-hold prep cooler food items that had been prepared the previous day, including three pans of sliced cheese and pans of bacon bits, bleu cheese, shredded cheese, butter, tartar sauce, salad greens, and cut onion.
    • (CORRECTED DURING INSPECTION): Cooked bacon for TPHC not marked with time removed from cooling for tracking 4-hour allowance.
    • (CORRECTED DURING INSPECTION): A number of food-contact surfaces were found in an insanitary state and required re-washing, including a significant number of metal hotel pans and lids with food residue, and items that were stored in plastic tubs with food debris and residue such as plastic storage jugs, funnels, plastic lids, squeeze bottles, and nozzles. A small number of pans were found not to be fully dry before being placed in storage.
    • Condensation in walk-in freezer observed to be collecting behind cooling unit and dripping onto cases of frozen chicken tenders (3) below.
    • A variety of non-food contact surfaces were found in an insanitary state, including multiple plastic tubs for holding wares/equipment with dried food debris on bottom, a bin holding single-use condiment cups and lids with debris accumulations, clean wares shelving containing food residue, debris, and an area of standing water, and exterior surfaces of the bulk dry ingredient bins and dish machine with excess food residue and debris.

    219 Central Bridge St, Wausau, WI 54401 (Inspection Date: 6/18/2024) Ten violations:

    • (CORRECTED DURING INSPECTION): Opened package of raw ground beef pucks found to have been placed directly on top of frozen custard cases in walk-in cooler.
    • (CORRECTED DURING INSPECTION): Bacon strips found to be inadquately hot-held in pan at temperatures ranging from approximately 90 to 115 F.
    • Opened package of ground beef pucks at top of burger prep cooler found to be improperly cold-held on cook line at approximately 48 F.
    • Staff is not adequately monitoring food temperatures during hot and cold holding.
    • Numerous food-contact surfaces were found to be in an insanitary state, including items held in tubs with deceased pests and food debris (metal pan lids, squeeze bottles, plastic jugs, plastic lids, measuring cups, a grill scraper, and condiment bottle nozzles). Small number of metal pans on clean storage shelf had flour-like substance on sides, edges, and rims.
    • Condensation resulting from non-sealed area of walk-in cooler wall observed to be dripping onto an opened case of butter and running down an approximate 6-foot length of the wall along which other product is stored.
    • Numerous non-food contact surfaces found to be in an insanitary state. Deceased ants were found in the bottoms of a majority of large plastic tubs that are used for holding clean equipment and wares, which are located near receiving door with gap. Tubs also contain accumulations of food debris. Areas on clean wares racks contain food debris and residue. Stainless steel shelf in prep area directly above hot holder unit and top surface of dish machine contain dried debris. Areas of floor under racks in walk-in cooler harbor residue and discoloration.
    • Numerous metal pans were found to be stacked together in an interlocking fashion without complete air-drying.
    • Corner of metal wall panel in walk-in cooler is peeling away from the wall, creating an uncleanable surface that accumulates moisture and causes condensation formation along wall.
    • Small gap exists at bottom-left corner of receiving door that fails to prevent entrance of pests.

    225577 Lilac Av, Wausau, WI 54401 (Inspection Date: 6/21/2024) Eight violations:

    • Temperatures of two hamburger patties to be served were found to be below the critical limit of 165 F for non-intact raw beef.
    • (CORRECTED DURING INSPECTION): Numerous non-shelf stable items at frozen custard prep area were found to be above the critical limit of 41 F, including raspberry sauce, strawberry sauce, peach topping, and cookie dough bites at approximately 48 F. Additionally, temperatures of portioned cups of coleslaw in the front-of-house cooler were found to range from 43 to 48 F. Items were within timeframe to be safely cooled.
    • Blue disposable gloves for handling of raw fish repeatedly observed to be in contact with cook line trash bin.
    • Date-marking absent on a number of required food items, including bacon strips, a large pan of lettuce, pans of sliced ginger and mayonnaise, a container of shake mix, and a small container of broccoli. Additionally a container of bleu cheese crumbles was incorrectly labeled with a 10-day use-by date.
    • (CORRECTED DURING INSPECTION): Bacon strips using time as a public health control for 4 hours is not properly marked or identified with a discard time.
    • Walk-in freezer has significant areas of condensation on ceiling that drip onto product cases and the floor near other stored product during defrost cycle.
    • Numerous non-food contact surfaces were found to harbor excess food debris, including bulk dry ingredient bin exteriors, multiple storage bins for cleaned wares and utensils, interior surfaces of stainless steel drawers for utensil storage, the stainless steel shelf for storing metal hotel pans, a container at front of house that holds disposable forks, and a bin for storage of sandwich buns. Additionally, ice machine interior had minor biofilm formations, two bins for storage of clean wares were found to harbor splashed water from handwashing activities, and the walk-in cooler floor had a moderate area of pooled water.
    • Proximity of the clean wares rack to the cook line handwash sink was observed to cause water from handwashing activities to splash onto cleaned wares and into their containers.

    Day’s Bowl-A-Dome

    • Address: 1715 Stewart Av, Wausau, WI 54401
    • Inspection Date: 6/26/2024. Four violations:
    • No air gap provided on ice machine.
    • The food prep table cutting board located in the kitchen is marred with many grooves, prohibiting adequate sanitization.
    • (REPEAT): Wiping cloth used for wiping surfaces at the bar was not being stored in a sanitizing solution.
    • The pizza oven tray liners were soiled.

    DC Everest Greenheck Fieldhouse

    • Address: 6400 Alderson, Schofield, WI 54476
    • Inspection Date: 6/11/2024. One violation:
    • An irreversible registering thermometer is not provided for measuring utensil surface temperatures in the hot water mechanical warewashing operation.

    Den Mar Tavern

    • Address: 601 W Thomas St, Wausau, WI 54401

    Inspection Date: 6/20/2024. Five violations:

    • (CORRECTED DURING INSPECTION): Shell eggs observed to be stored over numerous ready-to-eat food items including sliced potato and shredded cheese.
    • Date-marking was observed to be absent on a number of required food items including sliced cheese, shredded cheese, sliced pickles, sliced jalapeno peppers, a bowl of macaroni salad, raw fish filets, hardboiled eggs, tartar sauce bottles, a metal pot of potatoes, and a pan of bacon.
    • Consumer menu does not indicate which menu items are offered undercooked.
    • Numerous food-contact surfaces were found to harbor excess food debris and residue, including metal pot lids, plastic pan lids, volumetric pitchers, a strainer, a metal mixing bowl, and a large cooking pot with a deceased insect inside. Additionally, a baking sheet was found to have areas of rust, and a plastic mixing bowl was found to be damaged with gouges, permanent staining, and odor (discarded).
    • A number of non-food contact surfaces were found in an insanitary state, including storage tubs holding cleaned wares and single-use condiment cups with food debris on the bottom surface, and some interior surfaces of the ice machine with minor mildew formations.

    Halder Bridge Bar

    • Address: 133071 County Road C, Mosinee, WI 54455
    • Inspection Date: 6/25/2024. No violations.

    Hiawatha Sports Bar

    • Address: 713 Grant St, Wausau, WI 54403

    Inspection Date: 6/27/2024. Eight violations.

    • Concentration of chlorine sanitizer in spray bottle found to be significantly above the upper limit of 200 ppm.
    • Bar ice bin drain lacks a full air gap.
    • Numerous food items that require date-marking were found to have no labeling to indicate either the date of preparation or the date of discard, including multiple containers of shredded cheese, onion, tomato, lettuce, and sliced cheese. Additionally, labeled with past due dates were a container of pulled pork and a container of raw ground beef patties.
    • Numerous food contact surfaces were found to be in contact with food debris and residue, including a large number of metal and plastic hotel pans and lids, spatulas, meat tenderizers, scoops, a grater, and a pizza cutter.
    • (CORRECTED DURING INSPECTION): Two employees working in main food preparation area were observed to be lacking hair restraints.
    • (REPEAT): Prep table cooler in kitchen and sauce/condiment reach-in cooler observed to be lacking thermometers for ambient temperature monitoring.
    • Numerous non-food contact surfaces were found to harbor significant food debris and residue, including two bins used for holding large quantity of container lids, a plastic bucket used for holding an assortment of kitchen utensils, and prep table cross-bars. Additionally, a container for dry ingredient storage had a small deceased insect and significant food debris at the bottom and the top and exterior surfaces of bulk storage bins for sugar and flour were found with encrusted debris and residue accumulations.
    • Back door of kitchen found propped fully open, failing to prevent potential pest entrance. Screen door has gaps at bottom and sides, and screen has numerous holes.

    Hutch’s

    • Address: 4411 Stewart Av, Wausau, WI 54401
    • Inspection Date: 6/27/2024. Two violations.
    • The warewashing machine behind bar is not dispensing sanitizer automatically.
    • Cavity and exterior of microwave are soiled.

    King Buffet

    • Address: 1035 E Grand Av, Rothschild, WI 54474
    • Inspection Date: 6/7/2024 (Re-inspection). No violations.

    La Quinta Inn

    • Address: 1910 Stewart Av, Wausau, WI 54401
    • Inspection Date: 6/14/2024. No violations.

    Lisa’s at Lakeside

    • Address: 214857 County Road Y, Hatley, WI 54440
    • Inspection Date: 6/20/2024. Four violations.
    • (CORRECTED DURING INSPECTION): Holandaise sauce and the biscuit gravy have exceeded its date mark and must be discarded.
    • All ready to eat foods items held in all coolers need to be preoperly datemarked. (cole slaw in the upright cooler not date marked)
    • Thermometer for the cooler in the wait station is not accurate. Provide thermometers for coolers.
    • The hot water handle is leaking for the handwash sink in the kitchen. Repair or replace faucet.
    • Address: 157024 E Hamilton St, Wausau, WI 54403

    Inspection Date: 6/27/2024. Five violations:

    • Raw fish fillets found to be stored over ready-to-eat bacon strips in raw meat area of walk-in cooler.
    • (CORRECTED DURING INSPECTION): Staff is not monitoring sanitization temperatures of high-heat dish machine.
    • Date-marking observed to be absent on a number of food items that require it, including large pans of fish filets, containers of raw chicken, plated cake slices, plated macaroni and cheese entrees, and containers of diced potato, shredded cheese, sliced tomato and cucumber, portioned mayonnaise, lettuce, cooked pasta, mashed potato, potato salad, shrimp, and cooked beef rib.
    • (CORRECTED DURING INSPECTION): A number of foods were found not covered while in cold storage, including containers of raw chicken breast and wings (5), a bin of french fries, and a pitcher of tartar sauce.
    • Receiving door has gap at lower hinge side of approximately 1 x 0.5″ and a gap of approximately 18 x 0.2″ on the lower latch side that fails to prevent potential entrance of pests.

    Log Cabin

    • Address: 1522 Metro Dr, Schofield, WI 54476

    Inspection Date: 6/14/2024. Eight violations.

    • (CORRECTED DURING INSPECTION): Internal temperature of bacon strips and sausage links in hot-hold unit on cook line found to be below the lower critical limit of 135 F for hot-held items, due to uneven distribution within the container.
    • Ice bin at front-of-house wait staff prep area observed to be lacking a full air-gap to prevent contamination in the event of backflow.
    • Staff is not monitoring hot-hold temperatures for bacon strips and sausage links in hot-hold unit on cook line.
    • Numerous food items were observed to have past-due date-marks, including shredded mozzarella cheese, a container of fish butter, honey sriracha sauce, a container of corned beef hash, waffle batter, and condiment/sauce bottles (5). Additionally, date-marking was found to be absent on a number of required food items, including a large tub of chopped lettuce, condiment bottles (3), and fruit cups (12). Items were discarded as necessary.
    • Consumer menu has advisory statement but does not designate which foods are offered undercooked (steak, burger, and some egg items).
    • Numerous food-contact surfaces were observed to be insanitary, including metal pan lids stored in bin with food debris accumulations, counter-mount can opener with debris and residue build-up, squeeze bottle tops with residue, plastic hotel pans, containers, and lids with food debris, and ice machine scoop in standing water with mild grime build-up.
    • Numerous non-food contact surfaces were found to be insanitary, including lid of bulk flour bin with residue build-up, large gray bin for metal pan lids and hot holder cross-bars with debris accumulation, exteriors of cleaned plastic pans with old date-mark labels and adhesive residue, cleaned wares rack with rust spots, bins for various lids and cookware with accumulated food debris on handles and at bottom, slight mildew formations in ice machine compartment, ice scoop container with minor grime build-up and standing water, interior bottom surfaces of ice cream freezer with debris build-up, and the exterior surfaces of two food thermometers with residue accumulation.
    • Significant number of cleaned wares were found to be stacked together prior to air-drying, including metal hotel pans, plastic containers, and drinking cups.

    Mark’s Bar

    • Address: 211402 State Highways 49, Elderon, WI 54429
    • Inspection Date: 6/5/2024. No violations.

    The Mint Cafe

    • Address: 422 N 3rd, Wausau, WI 54403
    • Inspection Date: 6/19/2024. Two violations.
    • Consumer advisory is missing the disclosure statement. Must asterisk each item that maybe served undercooked. Items on the kids menu not allowed to be served undercooked. Update on the next menu printing.

    • The Person in Charge (PIC) is not monitoring the proper methods for rapidly cooling food. This is regarding the improper cooling procedure observed for the German potato salad.
    • The German potato salad was date marked 7/1 when it was prepared on 6/21.
    • Sterilite brand containers are not ANSI/NSF certified or approved by the department.

    Perla’s Place Bar & Grill

    • Inspection Date: 6/4/2024. Seven violations:
    • Store all medications in a different location other than the kitchen.
    • All ready to eat foods held in the cooler longer than 24 hrs needs to be date marked and used within 7 days.
    • Thermometer used in food establishment is not accurate to +/- [ ]°F.
    • A chlorine test kit is not available for checking sanitizer concentrations.
    • The splash guard inside the ice machine needs cleaning to remove mold.
    • The soda gun dispensing head needs cleaning to remove buildup of syrup.
    • Label all storage containers on the grill line as to contents. (oil, sauces, etc)

    Pine Valley Golf Course

    • Inspection Date: 6/12/2024. Two violations:
    • Thermometers in the bar cooler and the true undercounter cooler are old and no longer accurate.
    • House made bloody Mary mix was observed stored in a Simply Orange juice container.

    • Inspection Date: 6/21/2024. Three violations:
    • No consumer advisory provided post on the menu.
    • Remove all the duct tape from inside the freezer door. Not a cleanable surface. If needed replace freezer.
    • The screen door in the kitchen needs to be repaired so it closes tight.

    S & H Kelly Club

    • Address: 4810 Ross Av, Weston, WI 54476
    • Inspection Date: 6/26/2024. Three violations:
    • Wiping cloth used at the bar for wiping surfaces stored on sink and not in a sanitizer solution.
    • (REPEAT): An irreversible registering thermometer or thermolabel is not provided for measuring utensil surface temperatures in the hot water mechanical warewashing operation.
    • The handle of the Frigidaire freezer is broken and not easily cleanable.

    Sam’s Pizza of Schofield

    • Address: 5811 Business Highway 51, Schofield, WI 54476

    Inspection Date: 6/13/2024. Nine violations:

    • Raw bulk seasoned sausage in two metal pans stored directly over pepperoni in walk-in cooler.
    • Concentration of chlorine sanitizer in spray bottles found to be significantly over limit of 200 ppm when tested.
    • (CORRECTED DURING INSPECTION): Two cooler units were found to be operating above 41 F, with foods inside ranging from 44 to 49 F overnight. Food items found to be over 41 F include 12 portioned bleu cheese dressing cups, 5 gallon jugs and 2 pitchers of caesar dressing, 1 jug of thousand island dressing, 1 jug and 2 pitchers of ranch dressing, 1 jug of parmesan dressing, a pan of sliced cucumber, a pan of shredded vegetables, and 2 cases of chopped lettuce. All affected items were discarded.
    • Staff not monitoring sanitizer concentration, evidenced by two bleach spray bottles with concentrations significantly over 200 ppm. Test kits were onsite but not located near area of use. Staff unsure of which test kit to use for verification of bleach concentration.
    • Date-marking largely absent from required non-shelf stable food items, including sliced cucumber, shredded vegetable mix, portioned dressing cups, pans of sliced mushroom, onion, and green pepper, cooked ground beef, Canadian bacon, 22 containers of red sauce, one large metal container of sliced cheese, two tubs of garlic butter, and two pans of bulk raw sausage.
    • Numerous food-contact surfaces were observed to be visibly soiled, including two large metal strainers with food debris, significant number of sauce pails and lids stained and odorous (discarded), and utensils with food debris and residue such as measuring cups, tongs, spatulas, scoops, a shaker, and container lids. Additionally a large number of drinking cups were found to have been stacked together prior to air-drying, trapping water inside from washing actvities the night before.
    • Square plastic Rubbermaid containers and lids labeled for sauces such as Parmesan and pizza fry butter not made for commercial use and lack NSF/ANSI approval.
    • Numerous non-food contact surfaces found to be insanitary, including walk-in cooler storage racks with debris accumulations and adhesive residue, exterior surfaces of plastic red sauce containers with debris, and significant accumulations of dried food debris in several bins used to hold cleaned wares, utensils, and equipment.
    • Roll-up garage door found to have gap at bottom-right corner approximately 6 by 0.75 inches. Rear receiving door has gaps at bottom-left corner and at center of bottom surface, both of approximately 6 by 0.5 inches. Ant was observed entering building at this time.

    Sam’s Pizza of Wausau

    • Address: 111 Elm St, Wausau, WI 54401

    Inspection Date: 6/27/2024. Six violations:

    • Pan of raw ground pork sausage was observed to be stored over non-raw foods including mushrooms, pepperoni, sliced cheese, and red sauce.
    • Front-of-house ice bin found to lack a proper air gap for the prevention of contamination in the event of backflow.
    • A number of cleaning chemicals were found to be stored directly above food-contact pizza bags in dry storage area, including degreaser, delimer, surface cleaner, and quaternary sanitizier.
    • Date-marking was observing to be absent from numerous food items that require it, including multiple containers of Canadian bacon, grilled chicken, shrimp, jalapeno peppers, chicken bacon ranch sauce, peppers, diced salami, and sliced mushrooms, as well as several buckets of red sauce, portioned ranch cups, and pans of bulk raw pork sausage.
    • (CORRECTED DURING INSPECTION): Pizza toppings using time as a public health control for 4 hours not properly marked or identified with a discard time.
    • Various non-food contact surfaces and items were found to harbor excess food debris, including interior surfaces of the drinking glass cooler, multiple tubs for the storage of utensils and kitchen equipment such as metal pans, sharpening steels, strainers, scoops, spatulas, whisks, can openers, and other like items. Additionally, a tub for holding jugs of canola oil and the surrounding shelf area was heavily soiled with oil and food debris, the interior of the ice machine displayed formations of mildew, and a pail for bulk black pepper storage had significant encrusted residue on external surfaces.

    Shanghai Grill

    • Staff is not monitoring sanitizer concentration in kitchen and bar dish machines.
    • Date-marking found to be absent on a number of required items in prep table cooler, including pans of green pepper, red sauce, sausage, mushrooms, shredded cheese, onion, sliced cheese, portioned ranch dressing cups, tartar sauce cups, a container of soup broth, a tray of raw ground beef patties, and a tub of bacon.

Additionally the upper internal surface of the ice machine had minor mildew formations.

Additionally, a kitchen vent of approximately 12 sq. inches was found open to the outside with no screen or other method of pest prevention.

    Inspection Date: 6/4/2024. 15 violations:

    • Employee performing warewashing duties observed stopping to use personal cell phone and then returning to warewashing without washing hands.
    • (REPEAT): Foods in south prep table wells and cabinet found to be stored between 46 and 50 F overnight, including chili with beans, jalapeno peppers, several containers of cooked pasta, strawberry sauce, gravy, and sour cream. All were discarded in the presence of the inspector. The north (large) prep table was found to be insufficiently cooling foods in the wells and cabinet as well, at temperatures between 55 and 67 F, including sliced cheeses, bacon bits, feta, lettuce, sliced tomato, diced ham, mayonnaise, pepper, mushroom, and onion. These items were allowed to be cooled and used again due to timeframe.

    • Raw fish fillets on baking sheets were stored directly above a large bin of tortilla chips in walk-in cooler.
    • (CORRECTED DURING INSPECTION): Food items in large counter cooler were found to have been held at approximately 48 F beyond the timeframe to cool, including 12 plated lasagna slices, 19 plated fish filet dinners, 12 baked potatoes, 8 twice-baked potato dishes, 2 large containers of cooked pasta, and 7 sauce/condiment bottles.
