Norfolk Southern and its contractors botched the decision to blow open five tank cars and burn the vinyl chloride inside after last year’s disastrous derailment in East Palestine, Ohio, and trackside detectors that might have prevented the crash failed to accurately detect the temperature of a burning wheel bearing 20 miles (32.19 kilometers) beforehand, according to the National Transportation Safety Board.

New Cumberland, June 13. Fail. The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. Two male employees observed in the kitchen area, not wearing proper hair restraints, such as beard covers. Observed numerous, but not all food ingredient storage containers, shakers, bottles, in the entire facility, is not labeled with the common name of the food. Observed hose running from wall installed air-conditioning unit to hand-wash sink. Hose must be re-routed to not drain into the hand-wash sink. Exit screen door located in the rear exit area of the food facility has a gap around the entire perimeter of the door, and does not protect against the entry of insects, rodents, and other animals.
