CHARLTON — A Southbridge woman was killed in a head-on car crash on Southbridge Road after an incoming vehicle drove into her lane late Saturday, according to authorities.

The Worcester County District Attorney's Office of Joseph D. Early Jr. identified the victim as Leah Forrester, 51.

Three family members were also in the vehicle. They remain hospitalized.

A GoFundMe page was created for the victims' family.

A Charlton woman faces charges in connection with the crash.

Aphrodite Papoutsides, 26, of Charlton was arraigned Monday in Dudley District Court on counts of reckless homicide by motor vehicle, reckless driving and a marked lanes violation, according to Early's office.

Police said Papoutsides was traveling north on Southbridge Road (Route 169) around 9 p.m. when her Mercedes crossed the center line. Her vehicle crashed into a Honda Civic traveling south on the opposite side of the road.

The impact resulted in the death of the Honda driver, a 51-year-old woman from Southbridge, police said. Her passengers were transported to a hospital, according to police.

Police said Papoutsides was also transported to a hospital with injuries.

At the arraignment Monday, a Dudley District Court judge set bail at $7,500, Early's office said.

The judge also set conditions that required Papoutsides to not drive, to surrender her passport, to remain drug and alcohol free and to have no contact with the witnesses.

Early's office said that the prosecutor assigned in the case had asked for a $25,000 bail.

Papoutsides is set to return to court November 7.
