Violations include: Cockroach found in the refrigerator; black mold spots on cutting board; can of chicken broth had a hole on the surface and it was stored on the dry storage shelf; raw lamb chops are stored above ready-to-eat/cooked foods; no thermometers in the freezer or cooler; Heating lamp covered in grease; bottom of refrigerator needs to be clean; toilet door is removed from its structure; aluminum pans and vent hood covered in grease; light switch cover broken in men restroom and in the kitchen; kitchen dim due to lights not working; and food permit is expired.
