When they arrived, they learned that Maddox had been walking across the yard when he was hit by the bucket of a front-end loader operated by another worker. Rescue workers tried to resuscitate the 43-year-old, but they were unsuccessful.</p><h2>OSHA is Investigating the Fatal Accident at Dayton Sand and Gravel</h2><p>This incident is under investigation by the Occupational Safety and Health Administration (OSHA) but police say there's no indication of any criminal activity.
