An employee died Friday night in an industrial accident at a Mosaic plant in the Bradley area south of Mulberry, the Polk County Sheriff’s Office reported.

Jameka Williams, 29, a process operator, was working atop a piece of machinery when she slipped and fell into it, Sheriff Grady Judd said in a video posted Saturday. Williams died at the scene, Judd said.

Williams had been a Mosaic employee for nearly two years, company spokesperson Jackie Barron said.

“We join together to extend our deepest condolences to our colleague’s loved ones and fellow New Wales employees," Barron said in an email. "Counselors remain on site providing support and assistance to family and coworkers. As a multi-generational, close-knit company, this is a tremendous loss felt across Mosaic.  We continue to work with local authorities as the investigation into this incident is ongoing. Please keep all in your prayers at this difficult time.”

Representatives from the Occupational Safety and Health Administration arrived and are investigating the incident, PCSO said. An autopsy will be conducted this week.
