    Chardon women killed in Ohio Turnpike crash

    WASHINGTON TOWNSHIP - The Milan post of the Ohio State Highway Patrol is investigating a three-vehicle fatal wreck in which a Chardon woman died Monday.

    The crash occurred at 9:50 a.m. Monday, July 22, on the Ohio Turnpike, east of the Ohio 51 interchange in Washington Township, Sandusky County.

    Samantha Smith, 35, Chardon, died at the scene. She was transported by the Sandusky County Coroner’s Office to Mercy Health — St. Vincent Medical Center in Toledo.

    Tractor-dump trailer hit two barriers in westbound lane

    The preliminary investigation revealed a Kenworth T880 tractor-dump trailer, operated by Timothy D. Moulton, 58, Toledo, was traveling west on the turnpike when it drove off the right side of the roadway and struck a concrete barrier wall.

    The Kenworth T880 then crossed the westbound lanes and struck another concrete barrier wall, driving over it and into the eastbound lanes where it struck a Ford Escape.

    After striking the Ford, the Kenworth T880 continued across the eastbound lanes and struck a guardrail. It then struck a Kenworth T680 tractor-trailer, operated by Keo S. Thach, 43, Countryside, Il. The Ford, operated by Smith, continued a short distance before it caught fire.

    Moulton suffered serious injuries and was transported by air ambulance to Mercy Health — St. Vincent Medical Center in Toledo. St. Vincent had no condition on Moulton Tuesday.Thach was not injured as a result of the wreck.

    During the collision investigation, the eastbound lanes were closed for approximately five hours.

    The Ohio State Highway Patrol was assisted at the scene by the Ohio Turnpike and Infrastructure Commission, Harris-Elmore Fire Department, Sandusky County EMS, Sandusky County Coroner’s Office, Mercy Health Life Flight, Customs and Border Protection, Madison Motor Service, and Interstate Towing and Transport Specialist.

    The incident remains under investigation.
