HENDERSON, Ky. (WEHT) – Attorney General Todd Rokita’s office announced that criminal investigations into alleged Medicaid fraud has resulted in fraud charges against multiple individuals throughout the state, including three people in Vanderburgh County and one in Warrick County.

According to a release, Amy Nicole Mullins, 36, of Rockport, is charged with failure to make, keep or furnish records and obtaining a controlled substance by fraud or deceit. Mullins is accused of stealing medication prescribed from an Evansville nursing home.

Also in Vanderburgh County, Autumn Marie Duvall, 43, of Anderson, is charged with failure to make, keep or furnish records; obtaining a controlled substance by fraud or deceit; and possession of a narcotic drug. Duvall allegedly stole narcotic medication prescribed for two residents of an assisted living facility in Evansville. The release from Rokita’s office states one of the residents had already been discharged from the facility and reported the medication that she had brought to the facility when she was admitted could not be found when she was discharged.

Myah Alise Samples, 25, of Evansville was charged with failure to make, keep or furnish records; obtaining a controlled substance by fraud or deceit; and possession of a narcotic drug. Samples allegedly took narcotic pain medication prescribed to a patient in an assisted living facility in Evansville. Authorities say Samples admitted to taking the pills.

In Warrick County, Jessica Lynn Hallam, 39, of Evansville, was charged with three felonies related to the diversion of controlled substances from a hospital. Officials say Hallam was reported by co-workers after becoming outwardly impaired during her shift as a registered nurse at a hospital. During an investigation, Hallam allegedly admitted to stealing morphine, dilaudid and lorazepam from the hospital and falsifying her documentation for years.

According to a release, these investigations are part of a nationwide initiative led by the U.S. Department of Justice and the U.S. Department of Health and Human Services.
