    Restaurant inspections: June 20-25 critical violations

    The following restaurant inspections with critical violations were conducted by Richland Public Health between June 20 and June 25:

    OhioHealth Mansfield Hospital , 335 Glessner Ave., Mansfield, June 20. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical repeat). Observed no change during second reinspection. Correct by next reinspection. Presence of live insects (critical repeat). Observed multiple small flying insects in dishwashing room and in prep room. Slightly fewer insects observed during first reinspection June 10 but observed no change in number of insects during second reinspection on June 20. Facility provided both an action plan and cleaning schedule for the area of concern. Pest control has done a walk-through facility to target specific structural issues. Facility also had a company plan on addressing some of the structural issues onsite. Facility is to continue to send service reports to environmental health specialist (EHSIT) for review until August. Clean, disinfect and rid areas of pests by reinspection.

    Smokin' Bros BBQ , 1041 Park Avenue West, Mansfield, June 21. Food employee(s) did not wash hands when required (critical repeat, corrected during inspection). Observed employee not wash hands prior to donning gloves after touching cell phone. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed blade of the can opener and scissors used for food packages to have visible residue. Working containers of poisonous or toxic materials not properly labeled (critical repeat, corrected). Observed two spray bottles of chemicals in working containers to have not common name on the container.

    Cajunboi Concessions , 227 W. First St., Mansfield, June 22. Handwashing sink not accessible (critical, corrected during inspection). Observed a pitcher in hand wash sink.

    National Parkway Duke/Duchess/BP1186 , 1020 National Parkway, Mansfield, June 24. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Observed no change during reinspection. Correct by July 1. Presence of live insects (critical repeat). Observed several big and small black flying insects in the back storage, mop area and kitchen area near the drains. During reinspection observed no big fly insects, however did find small black flying insects in the cabinet near the ready-to-go cooler and within that cooler. Facility is to contact a licensed pest control operator, sanitize the areas and completely rid all evidence of pests by July 1 reinspection.

    Taco Bell #30674 , 1194 Park Avenue West, Mansfield, June 24. Person in charge did not ensure that employees observed foods as they were received (critical, corrected during inspection). PIC stated that temperatures are normally not taken on incoming product. Ensure all proper is checked for damages and that it is coming in at the proper temperature. Education was provided. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by reinspection July 1. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Observed no signed employee illness agreements. Ensure all employees read and sign employee illness agreements upon hire and these records are maintain where they are accessible for EHSIT to review. PIC had signed at time of inspection. Equipment food-contact surfaces or utensils are unclean (critical, corrected). Observed spatulas to be unclean when stored in the clean dish area. TCS foods not being cold held at the proper temperature (critical, corrected). Observed sour cream on the make line to have an internal temperature of 48 degrees. A time without temperature policy was created and implemented during inspection. PIC voluntarily discarded one-half container of sour cream. Presence of live insects (critical). Observed various live insects throughout the facility, namely near the warewashing area and under the lobby soda machine. Facility had pest control on site on June 21. Facility is to contact a licensed pest control operator, rid all evidence of live insects, completely clean and disinfect the areas effected, and have summary reports available for EHSIT review by reinspection July 1. Food contact surfaces not easily cleanable (critical, corrected). Observed plastic pan to have visible stained residue, rendering the surfaces no longer smooth, durable, non-absorbent or easily cleanable. PIC voluntarily discarded the pan. No air gap or approved backflow prevention device on the plumbing system (critical). Observed no air gap on the rethermilizer; it was observed to be in contact with the floor. Correct by reinspection July 1.

    Domino's Pizza , 625 Lexington Ave., Mansfield, June 25. PIC unable to demonstrate knowledge by having no critical violations (critical repeat). Observed no change during reinspection. Correct by next reinspection July 2. Presence of live insects (critical repeat). Observed big and small flying insects throughout the facility, concentrated in the following areas: mop sink, warewashing sink (black bins), by the back door and near the make line. Observed no change during reinspection. Facility had June 21 pest control report available for review. Facility is to continue working with pest control in order to completely rid facility of evidence of insects. Correct by reinspection July 2. Presence of live rodents (critical repeat). Observed rodent droppings along the handwashing sink wall in the back area. Observed no change during reinspection. Facility is to continue working with pest control in order to completely rid facility of evidence of rodents. Correct by reinspection July 2.

    Chipotle Mexican Grill #0489 , 764 N. Lexington Springmill Road, Mansfield, June 25. TCS foods not being cold held at the proper temperature (critical, corrected during inspection). Observed cut lettuce held on prep line to have an internal temperature between 44-52 degrees. PIC discarded lettuce. Ensure cut lettuce is held at an internal temperature of 41 degrees or below.

    Camp Otyokwah , 3380 Tugend Road, Butler, June 25. Improper washing of hands and arms (critical, corrected during inspection). Food worker observed touching dirty spiggot handle after washing hands. Proper procedure reviewed with worker to ensure clean hands do not touch contaminated surfaces. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected). Twelve pitchers of lemonade observed uncovered and subject to sneeze, splash, dirt or dust in walk-in cooler. Lemonade was located for serving. Food equipment surfaces not cleaned at required frequency (critical, corrected during inspection). Probe thermometer observed used to take internal temperature of chicken nuggets without first sanitizing surface. Proper procedure was discussed.
