JULY 2
Little Caesars, 7332 Staples Mill Road – The following violations were reported during a routine inspection: dried food debris found on can opener; temperature of multiple containers of pizza sauce made the previous night are elevated; time is not being tracked for pizzas in staging racks; sanitizer chemical is dispensing too high at over 400 ppm; multiple pizza sauce containers are covered and stacked in make table reach-in; single-use items are stored on a greasy unprotected rack in front storage; observed dust and debris on dough mixer, walk-in storage racks, dry rack, equipment rack and pizza oven; there is dust buildup on air vents within walk-in fan, ceiling vents and panels; cement floor by mop sink isn’t smooth and easily cleanable.
The Local Cup, 7700 East Parham Road – No violations reported during a routine inspection.
Wendy’s, 9116 West Broad Street – The following violations were reported during a routine inspection: temperatures of raw patties in ice bath and sliced tomato on the line are elevated (both may be stacked too high); several units are not in use on the cook line; fryer cabinet doors are loose; there is leak at the hand sink next to drive-thru window; water filtration system is leaking; floors and pipes behind fryers are dirty; grease buildup found on grease trap exterior; observed small nail holes in walls throughout kitchen.

Teppanyaki Grill & Buffet, 9043 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit/diarrhea; pot found in hand sink in dish room; raw chicken stored over cooked pork ribs in reach-in cooler on cook line; raw chicken stored over raw beef and fish in walk-in cooler; can opener is soiled; ice machine panel in wait station near party room is dirty; no sanitizer is detected in in-use dish machine; temperatures of several containers of meat and noodles in walk-in cooler are elevated; temperatures of chicken feet, chicken and beef in prep units are elevated; person in charge couldn’t provide written time control policy; spray container of degreaser in dry storage area is unlabeled; temperature of clams in reach-in cooler near grill is elevated; observed several TCS foods throughout the kitchen in deep containers; box of mussels is thawing on floor in front of grill; thermometers on the outside of the walk-in units are broken; cutting board used to chop cabbage has deep scratches and black staining; torn gasket and torn air curtain found in walk-in cooler near dry storage area; food cart next to mop sink is broken; wire racks in walk-in units are chipped and rusted; plastic shelf liner in walk-in cooler is broken; wire racks in dry storage area, wire racks in refrigerated unit at sushi station, and fryer cabinet are dirty; faucet with spray hose attached doesn’t have a dual check valve installed; women’s restroom doesn’t have a covered trash can; floor tiles in wait station near bathrooms are cracked; ceiling vents in dish room are rusty; screen door is torn; observed grease buildup on hood, pipes and floor behind cook line equipment.
Wood and Iron Short Pump LLC, 12288 West Broad Street – The following violations were reported during a routine inspection: uncovered drink cup found on dish storage shelf; hand sinks are blocked by a rolling cart and are storing knives and other utensils; paper towel dispenser is empty; automatic soap dispenser isn’t working; menu doesn’t contain an asterisk linking fried egg containing meal to the consumer advisory; three spray bottles containing a liquid blue chemical in the bar are unlabeled; reach-in prep fridge has a torn gasket; cutting boards throughout facility are stained with deep scratches; foil is lining bottom shelf of prep table; rubber linings found on bar shelf; walk-in freezer racks, beer chest bottoms, and behind and around cooking equipment are dirty; mop isn’t hung to air dry.
Pho 1 Grill, 201 Towne Center West Boulevard – The following violations were reported during a routine inspection: paper towel dispenser is empty; hand sink in kitchen corner is blocked by empty cardboard boxes; shelled eggs are stored over cabbage in walk-in refrigerator; employee is washing disposable latex food gloves; sanitizer test strip measured 0 ppm after dishwasher cycle; interior of ice machine is dirty; several products prepped and in containers are not date marked; bag of frozen chicken is stored directly on the floor; cabbage head found in a grocery bag instead of food grade safe packaging; several knives are stored in the crevice between prep tables; shelf holding take-out containers is dirty; two fryer baskets are in disrepair; observed cling wrap, cardboard and foil liners throughout kitchen; cutting boards throughout kitchen are scratched; test strips are expired; dishes on drying rack are still dirty; walk-in fridge and freezer racks, racks holding clean dishes, dish holders for dishwasher and spaces in between equipment are dirty; dumpster area is surrounded with trash and loose debris; mop isn’t hung to air dry; kitchen floors are dirty and slippery from grease.

Maya Latin Kitchen, 353 East Williamsburg Road – The following violations were reported during a routine inspection: employee chopping cilantro has only one glove on and the ungloved hand touched the cilantro; two oil bottles on the cook line are unlabeled; green and brown cutting boards are heavily scratched.
Chick-fil-A, 1529 North Parham Road – The following violation was reported during a routine inspection: temperature of raw chicken in breading station and chicken storage reach-in is elevated.
JULY 8
Rose Marie Inn, 8923 Patterson Avenue – The following violations were reported during a follow-up inspection: pulled pork in reach-in cooler is past its expiration date; long white cutting board is heavily scratched.
N&W Kosher Catering, 5403 Monument Avenue – The following violations were reported during a routine inspection: there isn’t a written time control policy for cut tomatoes on the counter; dried food stains found on cabinets and walls throughout the kitchen area; observed food debris on reach-in cooler door gaskets.
Starbucks inside Publix, 5400 Wyndham Forest Drive – The following violations were reported during a routine inspection: left side of two-door reach-in unit has a torn gasket; paper towels are being used as a liner to dry dishes instead of a smooth, cleanable and nonabsorbant drying rack.
Dugout Bar & Grille, 1510 Mountain Road – The following violations were reported during a routine inspection: temperatures of chicken and cheese in rear reach-in fridge are elevated; menu doesn’t contain a consumer advisory for burgers that are made to order; cutting boards on prep line and drying rack are scratched and stained; an air gap between the water supply inlet and the flood level rim of the three compartment sink may not be less than one inch; rear wall and floors in back food storage area are not smooth, nonabsorbant and easily cleanable.

Rapid Pizza, 1592 Mountain Road – The following violations were reported during a routine inspection: staff drink found on cutting board at cook line; spinach is draining in a strainer in back hand sink; both hand sinks are missing hand soap and paper towels; raw shell eggs stored over cucumbers in walk-in cooler; diced tomatoes and marinara made yesterday are not date marked; two chemical spray bottles are unlabeled; chemical cart next to hand sink and wall under mop sink are dirty; ceiling vents are dusty.
Plaza Azteca Mexican Restaurant, 12020 West Broad Street – The following violations were reported during a routine inspection: person in charge couldn’t provide written procedures for cleaning up vomit/diarrhea; open canned drink found on back prep table; observed an employee work with raw meat and use the same gloved hands to handle other cooking equipment and dishes; hand sink at bar doesn’t have paper towels or hand soap; bar hand sink is being used as a dump sink; chlorine test strip tested 0 ppm on dishwasher; large ice machine in kitchen is dirty; broth in the walk-in refrigerator is past its expiration date; cleaning chemicals are stored on the same shelf as food ingredients; spray bottle with blue liquid is unlabeled; open tortilla shells found in a grocery bag instead of a food grade bag; cutting boards throughout kitchen are deeply scratched and stained; several items/areas are dirty (large ice machine, interior of refrigerated prep units throughout kitchen, stove line under hood, fryer baskets, lower prep shelves, vent guards and storage racks in walk-in refrigerators, drying racks, dry goods racks, ceiling vents throughout kitchen, and black crates holding food items); sink in back kitchen has a leak.
