    Beebe Healthcare opens Coordinated Care Center for post-acute visits

    LEWES — Beebe Healthcare has opened a new Coordinated Care Center, 1305 Savannah Rd., to bridge the gap between inpatient and crucial follow-up care for patients without a primary care provider being discharged from the Margaret H. Rollins Lewes Campus.

    These transitional visits are vital to avoid rehospitalization and connect patients with a primary care provider for continued support, the hospital said in a release.

    “This is a great step forward in the value of care that we provide to Sussex County,” Danielle Socrates, vice president of value-based care operations and transformation at Beebe, said. “This center will offer the right care for the patient at the right time, leading to lower costs for them by avoiding unnecessary emergency department visits and hospital readmissions. This center will provide value in our system, ensuring quality care at the right time and place.”

    Patients are given an appointment at the center prior to discharge; it does not accept walk-ins.
