    Richland County restaurant inspections: June 28 - July 1 critical violations

    The following restaurant inspections with critical violations were conducted by Richland Public Health between June 26 and July 1:

    Beautiful Foods LLC, 1393 Lexington Ave., Mansfield, June 26. Person in charge (PIC) unable to demonstrate knowledge by having no critical violations (critical, corrected during inspection). PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected). PIC stated that foods are being left on the grill until they are 70 degrees then are being taken to the walk-in cooler. Ensure all foods are being transferred into the walk-in cooler once they have reached in internal temperature of 135 degrees and are not being left out to cool. Person in charge did not ensure that employees observed foods as they were received (critical, corrected). PIC stated that temperatures are no longer being taken when foods are received. Refrigerated, ready-to-eat, TCS foods not properly date marked (critical, corrected). Observed house-made dressings made more than 24 hours prior to inspection with no date mark. Missing and/or incomplete allergen declaration(s) (critical repeat, corrected). Observed cole slaw (egg/dairy) and dried papaya/pineapple (sulfites) to have no allergen statements on the packages. Food contact surfaces not easily cleanable (critical, corrected). Observed scissors that are not easily separable that are used to open foods packages. PIC removed scissors from service.

    TJ's Bar & Grill, 777 Lexington Ave., Mansfield, June 27. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC could not state cooling time and temperature parameters. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed breadcrumbs uncovered on the make line. Food not properly protected from contamination by separation, packaging, and segregation (critical, corrected). Observed raw fish and burgers to be stored where potential cross contamination may occur. TCS foods not being hot held at the proper temperature (critical, corrected). Observed macaroni and cheese in the hot holding unit to have an internal temperature of 126-199 degrees throughout. This product was stored in a shallow hotel pan with very little water at the bottom of the steam well. Improper storage of poisonous or toxic materials (critical, corrected). Observed soap and lotion to be stored where potential contamination may occur. Food contact surfaces not easily cleanable (critical, corrected). Observed rust on a knife in the clean dish drawer, rendering it no longer smooth, durable, non-absorbent or easily cleanable. PIC voluntarily discarded knife.

    Line Star Enterprise LLC dba The Electric Company, 1400 W. Fourth St., Mansfield, June 27. PIC not ensuring employees are properly sanitizing equipment and utensils and monitoring of sanitizer parameters (critical, corrected during inspection). Due to facility knowingly having no running water available and being unable to provide alternative means, PIC was unable to ensure employees had the ability to wash dishes and equipment properly. Education was provided and the water was turned back on during inspection. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by reinspection July 5. Person in charge did not ensure employees were effectively cleaning their hands as required (critical, corrected). PIC was unable to ensure employees had the ability to wash hands properly due to no running water. Handwashing sink not accessible (critical, corrected). Observed handwashing sink in the back area of the facility to have equipment stored on/in the sink, rendering the sink inaccessible. Presence of live insects (critical). Observed small crawling insects to be in the warewashing area. Facility is to contact a pest control operator, remove all evidence of insects, completely disinfect the area and have pest control reports available for environmental health specialist (EHSIT) review during the July 5 reinspection. Manual wash solution not being maintained at 110 degrees or above (critical). Observed no hot water available for the warewashing area. Water under pressure is not available to all fixtures as required (critical, corrected). Observed no running water available at the facility, therefore there was not enough water velocity to allow for proper handwashing or washing. Facility is to ensure that there is running water at all times during operation. Water source and/or hot water capacity not sufficient (critical, corrected). Observed no running water available at the facility. Education about the importance/requirement of having water was provided and water was turned back on during inspection.

    Springmill Drive-In Theater, 1040 Springmill St., Mansfield, June 27. Person in charge did not ensure employees were effectively cleaning their hands as required (critical, corrected during inspection). Observed several times where employees were not washing hands when required or at the proper place. PIC unable to demonstrate knowledge of food temperatures and times for safe cooking (critical, corrected). PIC was unable to state proper cooking temperature of foods. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected). PIC could not state proper cooling parameters for cheese sauce. PIC unable to demonstrate knowledge by having no critical violations (critical). Correct by reinspection on July 5. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected). PIC was unable to state proper cleaning frequencies for items' food contact surfaces held without temperature. Ensure these utensils (tongs, pizza roller, fryer baskets, spatulas, etc.) re cleaned at a four-hour cleaning frequency if not held at temperature. Employees are not informed in a verifiable manner of their responsibility to report information about their health (critical, corrected). Observed no employee illness agreements on site. Food employee using a utensil more than once to taste (critical, corrected). Observed employee use hands to pick pieces of popcorn from the popcorn machine and eat them. Food employee(s) not washing hands in approved handwashing sink or automatic handwashing facility (critical, corrected). Observed employee attempt to wash hands at the warewashing sink. Food employee(s) did not wash hands when required (critical, corrected). Observed employee not wash hands when changing tasks and before donning gloves. Working containers of poisonous or toxic materials not properly labeled (critical, corrected). Observed working container of sanitizer to be unlabeled. Presence of live insects (critical). Observed large flying insects throughout the facility. Facility is to rid evidence of insects, thoroughly clean the area, contact a licensed pest control operator and have PCO reports available for review by EHSIT at reinspection July 5. Presence of live rodents (critical). Observed two live rodents and droppings within the facility. Droppings were observed in the cabinets when you first enter the facility. Also observed nesting-like materials under/near the hot water heater and the dry storage area. Facility is to rid evidence of rodents, thoroughly clean and sanitize the area, contact a licensed pest control operator, and have PCO reports available for review at reinspection.

    Circle K #4705374, 2094 Park Avenue West, Mansfield, June 27. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed OFF insect repellent and shampoo stored directly amongst consumables (Rolaids, Liquid IV) and directly above bottled beverages. PIC relocated to separate area during inspection.
