    News of public record: Guernsey County restaurant inspections

    The following are restaurant inspections with critical violations, as conducted by the Guernsey County Health Department from Jan. 22 to Feb. 2.

    Jan. 22

    Subway Byesville, 209 E. Main St., Byesville. The handwashing sink as food accumulations collected in the drain. Knives were not being properly washed or rinsed before being placed in the sanitizer solution.

    Jan. 23

    Hondro's Market, 232 Dewey Ave., Cambridge. Follow-up inspection. The person in charge (PIC) was unable to demonstrate knowledge by having no critical violations. The facility does not have a verifiable employee health reporting agreement on file. Packaged one pound portions of ground raw pork were in bags labeled not for sale indicating they are custom processed. Panini grill has food buildup. A large pile of corroded pennies on the prep sink drainboard. Broiler is dirty. Beverage dispensing machine nozzles have accumulations of syrup and mold. Loctite adhesive, isopropyl alcohol, 3-in-1 oil, and various other chemical products were stored with or above food, equipment, and utensils. Spray bottles of unknown liquid chemicals are not labeled with the common name of contents. Sub sandwiches prepared and packaged in-house are not labeled with ingredients or allergens.

    Jan. 24

    Brook Intermediate, 58601 Marietta Road, Byesville. Employees present at the time of inspection were not able to produce a verifiable employee health reporting agreement.

    Trail Family Diner, 320 Dewey Ave., Cambridge. The facility does not have a verifiable employee health reporting agreement on fil. The internal temperature of sliced tomatoes, cut lettuce, and cheese slices registered 48 to 50 F; must be at 41 F or below to prevent bacteria growth.

    Jan. 25

    Salt Fork Lodge - Dining Room, 14755 Cadiz Road, Lore City. Employees are not informed in a verifiable manner of their responsibility to report information about their health. US Foods bakery products were not labeled. The rinse temperature on the dishwasher registered 153 F and the final rinse temperature registered 177 F; must reach 160 F and 180 F respectively to be properly sanitized. Shredded and sliced cheeses under the grill registered 43 to 44 F; must be held at 41 F or below.  The water temperature at the prep sink registered 55 F.

    Jan. 25

    Sally's Short Cut Store, LLC, 71990 Old 21 Road, Kimbolton. The PIC was unable to demonstrate knowledge by having no critical violations. Several repeat critical violations were observed: date marking, allergen labeling and proper holding temperatures of food. Employees are not informed in a verifiable manner of their responsibility to report information about their health. Cheeseburger registered 129 F and pizza slices registered 126 to 129 F; must be held at 135 F or above. Sloppy joe meat in the deli case was dated use by Jan. 20 and was not discarded. TCS food must be used or discarded within 7 days. Some deli meats in the prep cooler and deli case were not date marked. Several items in the facility did not have allergens listed on the package − pizza and salads (cold/hold), brownies, and chocolate-covered marshmallows. This violation of missing/ improper labeling and undeclared allergens has been documented at least 18 times since 2014 and continues to be out of compliance. This matter is therefore being forwarded to the environmental health director for corrective action.

    Jan. 29

    Long John Silvers, 1821 Wheeling Ave., Cambridge. Employees are not informed in a verifiable manner of their responsibility to report information about their health.  An employee did not wash hands after coming inside after a break. Coffee pot was put on the shelf in the back storage room with coffee, filter and coffee grounds still inside.

    Wendy's, 2180 E. Wheeling Ave., Cambridge. Several TCS items were above 41 F − asiago cheese, ghost pepper cheese and lettuce registered 47.7 to 49.7 F. Vanilla frosty mix in machine registered 44 F. TCS food must be held at 41 F or below.

    McDonald's, 2351 E. Wheeling Ave., Cambridge. The PIC was unsure how long in-use utensils were sitting out and employees could not verify when items started being used. The temperature of the dish machine registered 87 F initially and then registered 110 to 114 F; it must be at least 120 F for dishwashers using chlorine as a sanitizer to ensure grease is broken down and equipment is properly sanitized. Grease buildup in the drain compartment of the machine. The detergent dispenser on the dish machine was empty.

    Jan. 30

    T&J Food Mart, 10568 Clay Pike Road, Derwent. The piercing portion of can opener and cutting portions of the potato slicer had excessive accumulations of food residues and debris. An opened package of deli loaf in the lunchmeat case had a date mark of Jan. 10. TCS foods prepared and packaged in-house and offered for customer self-service in retail cases are not date marked as required. Cut melons are not date marked.

    Jan. 31

    Creno's Pizza Cambridge, 224 Dewey Ave., Cambridge. A spray bottle of sanitizing solution was stored with food products and utensils.

    Pure Love Bakery, 142 S. Second St., Byesville. Follow-up inspection. Unnecessary persons were present in food preparation, food storage, and ware washing areas. Butter was stored at room temperature; must be 41 F or less to avoid pathogen growth.

    Feb. 1

    Valley Township VFW Post #8040, 11910 Clay Pike Road, Buffalo. The facility does not have a verifiable employee health reporting agreement on file. The handwashing sink in the kitchen is covered by wooden board to provide for additional counter space. Raw bacon was stored above ready-to-eat items. Cut tomatoes, cooked sausage patties, and other refrigerated, ready-to-eat, TCS foods were not date marked. A spray bottle of liquid chemicals was without a label bearing the common name of its contents.

    Feb. 2

    Southeastern Ohio Regional Medical Center, 1341 Clark St., Cambridge. Sliced tomato, cut leafy greens, and house-made dipping sauce had internal temperatures of 69 F, 66 F, and 63 F respectively. TCS foods must be cold-held at 41 F.
